摘要
目的探究老年高血压肾病患者采用贝那普利联合苯磺酸左旋氨氯地平治疗对血压控制、氧化应激反应及肾功能的影响。方法选择2020年6月—2022年8月本院收治的86例老年高血压肾病患者为研究对象,按随机数字表法分为两组,每组43例。对照组采用盐酸贝那普利治疗,观察组在此基础上采用苯磺酸左旋氨氯地平治疗,均持续治疗4周。比较两组患者的血压水平、氧化应激反应、肾功能及不良反应发生情况。结果观察组治疗后的舒张压、收缩压分别为(76.91±2.50)mmHg、(129.26±3.13)mmHg,均低于对照组,组间差异有统计学意义(P<0.05)。观察组治疗后的超氧化物歧化酶水平为(72.69±7.43)U/L,高于对照组,脂质过氧化物、丙二醛水平分别为(65.43±5.36)μmol/L、(17.76±2.39)μmol/L,均低于对照组,组间差异有统计学意义(P<0.05)。观察组治疗后的血浆白蛋白水平为(36.35±3.78)g/L,高于对照组,血肌酐、血尿素氮水平分别为(64.43±3.51)μmol/L、(4.45±0.76)mmol/L,均低于对照组,组间差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论老年高血压肾病患者采用贝那普利联合苯磺酸左旋氨氯地平治疗可有效控制血压,减轻氧化应激反应,促进肾功能改善,且安全性高。
Objective To investigate the effects of benazepril combined with levamlodipine besylate on blood pressure control,oxidative stress response and renal function in elderly patients with hypertensive nephropathy.Methods 86 elderly patients with hypertensive nephropathy admitted to the hospital from June 2020 to August 2022 were selected as the research objects and were divided into two groups according to random number table method,with 43 cases in each group.The control group was treated with benazepril hydrochloride,and the observation group was treated with levamlodipine besylate on this basis.Both groups were treated for 4 weeks.The blood pressure,oxidative stress response,renal function and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the diastolic blood pressure and systolic blood pressure in the observation group were(76.91±2.50)mmHg and(129.26±3.13)mmHg,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the superoxide dismutase level in the observation group was(72.69±7.43)U/L,higher than that in the control group,and the lipid peroxide and malondialdehyde levels were(65.43±5.36)μmol/L and(17.76±2.39)μmol/L,respectively,lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the plasma albumin level of the observation group was(36.35±3.78)g/L,higher than that of the control group,and the serum creatinine and blood urea nitrogen levels were(64.43±3.51)μmol/L and(4.45±0.76)mmol/L,respectively,lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Benazepril combined with levamlodipine besylate can effectively control blood pressure,reduce oxidative stress and improve renal function in elderly hypertensive nephropathy patients with high safety.
