MSCT、MRI检查对脑神经胶质瘤诊断及其组织病理学分析 被引量:11

摘要 目的旨在探讨多层螺旋扫描(MSCT)、磁共振(MRI)检查对脑神经胶质瘤诊断及其病理学特点。方法选取我院2017年3月-2018年4月收治的脑神经胶质瘤患者78例为研究对象,78例患者均进行MSCT与MRI检查,比较CT、MRI检查对脑神经胶质瘤的检出率及诊断符合率,总结MSCT、MRI检查中脑神经胶质瘤的影像学表现,分析其病理学特点。结果MSCT检查对脑神经胶质瘤检出率及诊断符合率分别为100.00%、97.43%,MRI检查对脑神经胶质瘤检出率及诊断符合率分别为100.00%、98.71%,两种检查方式对对脑神经胶质瘤检出率及诊断符合率比较,差异无明显差异(P>0.05);MSCT平扫图像表现为低密度或混杂密度肿块者64例,27例患者占位效应较轻,无强化或强化程度低,31例患者占位现象较重,强化明显,少数患者囊中有瘤,瘤中有囊,本质区域强化显著,囊壁呈现不规则强化。MRI图像扫描中多数患者可见肿瘤含实性囊性坏死小肿块,26例患者T1WI序列呈现低信号、T2WI高信号,34例患者表现为长T1、T2信号,信号不均匀或较为混杂,增强扫描多呈现轻-中度不均匀强化。肿瘤细胞为神经胶质及星形细胞,内存在钙化,合并瘤内出血,囊变区域、瘤周组织呈现炎性反应状态。结论MSCT、MRI检查均可有效检出诊断正确诊断脑神经胶质瘤,脑神经胶质瘤多为囊变实性肿块和壁结节的囊性肿块,常合并出血、钙化。 Objective To investigate the diagnostic and pathological characteristics of glioma by multi-slice spiral CT(MSCT)and magnetic resonance imaging(MRI).Methods 78 patients with glioma admitted to our hospital from March 2017 to April 2018 were selected as the study subjects.All 78 patients underwent MSCT and MRI examinations.The detection rate and diagnostic coincidence rate of CT and MRI for brain glioma were compared.The imaging manifestations of MSCT and MRI for brain glioma were summarized and the pathological characteristics were analyzed.Results The detection rate and diagnostic coincidence rate of MSCT for glioma were 100.00%and 97.43%respectively.The detection rate and diagnostic coincidence rate of MRI for glioma were 100.00%and 98.71%respectively.There was no significant difference in the detection rate and diagnostic coincidence rate between the two methods(P>0.05).MSCT plain scan showed 64 cases of low density or mixed density masses,27 cases had slight occupancy effect,no enhancement or low degree of enhancement,31 cases had serious occupancy and obvious enhancement,a few patients had tumors in the cysts,cysts in the tumors,significant enhancement in the essential area,irregular enhancement in the cyst wall.In the MRI image scanning,most of the patients showed small tumor with solid cystic necrosis.26 patients showed low signal on T1WI and high signal on T2WI,34 patients showed long signal on T1 and T2,and the signal was not uniform or mixed.Most of the enhanced scans showed mild to moderate non-uniform enhancement.The tumor cells were glial and astrocyte,with calcification,intracellular hemorrhage,and inflammatory response in cystic lesion area and peritumor tissue.Conclusion MSCT and MRI can effectively detect and diagnose gliomas.Most gliomas are cystic solid masses and mural nodules with hemorrhage and calcification.
