摘要
目的:探讨降钙素原(PCT)、白细胞计数(WBC)、中性粒细胞比例(Neu%)在新生儿晚发型败血症中的诊疗价值。方法:回顾性分析2017年1月-2019年1月我院新生儿科的住院患者,分晚发型败血症组、一般细菌感染组和未感染组,分析PCT、WBC、Neu%在各组的情况。结果:PCT用于诊断败血症及与一般感染组时受试者工作特征(ROC)曲线下面积(AUC)均最大,分别为0.987(P<0.05)和0.941(P<0.05),灵敏度及特异度均高于WBC及Neu%;WBC诊断一般感染时AUC为0.981(P<0.05),灵敏度为100%,特异度为86.7%,均高于PCT及Neu%;Neu%在诊断败血症及与一般感染鉴别时AUC、灵敏度及特异度均居中。结论:PCT、WBC、Neu%可指导诊断晚发型败血症,三者中PCT诊断败血症时效能最高,WBC诊断一般细菌感染时效能最高,Neu%诊断效能居中。
Objective:To investigate the clinical application value of serum procalcitonin(PCT),white blood cell count(WBC)and neutrophil percentage(Neu%)in neonatal late-onset sepsis.Methods:Retrospective analysis was conducted on the hospitalized patients in the neonatology department of Renmin Hospital of Wuhan University from January 2017 to January 2019.A total of 163 infants are divided into control group,general bacterial infection group and sepsis group.PCT,WBC,and Neu%of these three group are compared.Results:The area under ROC curve(AUC)of PCT in the diagnosis of sepsis and the differentiation from the general infection group were 0.987 and 0.941,both of them were higher than that respectively of WBC and Neu%.The sensitivity and specificity were higher than that respectively of WBC and Neu%too(all P<0.05).In the diagnosis of general infection,the AUC of WBC was 0.981,the sensitivity was 100%,and the specificity was 86.7%,both higher than taht of PCT and Neu%(all P<0.05).The AUC,sensitivity and specificity ofNeu%were all in the middle in the diagnosis of sepsis and the differentiation from general infection.Conclusion:PCT,WBC,and Neu%contribute to diagnose late-onset sepsis.The diagnostic value of PCT in late-onset sepsis is the highest.In general bacterial infection,WBC has the highest diagnos-tic efficiency,the value of Neu%is in the middle of PCT and WBC.
