摘要
《黄帝内经》认为肝与目在生理与病理上均相关,记载了许多肝与目相关的论述,如"肝经上连目系""肝气通于目,肝和则目能辨五色矣""肝受血而能视"等,因而后世医家多从肝论治眼科疾病。葡萄膜病是以黄仁为病变中心的疾病,但在症状表现上却以瞳神最为明显,故葡萄膜病常与中医学瞳神紧小、瞳神干缺相关。急性虹膜睫状体炎、慢性虹膜睫状体炎、中间葡萄膜炎、交感性眼炎、葡萄膜大脑炎、化脓性葡萄膜炎等是常见的葡萄膜病,病因病机均较为复杂,临床治疗难度大,其发病多与肝胆火盛,黄仁受灼,或湿热内蕴,熏蒸黄仁,或热毒伤阴,虚火上炎或外伤等有关。通过查阅古籍文献,对常见葡萄膜病病因病机进行分析,发现葡萄膜病的发生发展多与肝相关,在治疗上可从肝论治葡萄膜病,文章探讨了从肝论治葡萄膜病的基本原理。
Yellow Emperor′s Internal Classic believes that the liver and the head are physiologically and pathologically related,and records many discussions about the liver and the head.Therefore,the medical doctors of later generations mostly treat the eye diseases from liver.Uveitis is a disease with yellow kernel as the center of the disease,but the symptoms are most obvious with the pupils.Therefore,uvea disease is often related to the tightness of the pupils and the lack of pupils.Acute iridocyclitis,chronic iridocyclitis,intermediate uveitis,sympathetic ophthalmia,uveitis and suppurative uveitis are common uvea diseases,and the etiology and pathogenesis are more complicated.The clinical treatment is difficult,and its incidence is mostly related to hepatobiliary fire,Huang Ren suffering from burning,or damp heat content,fumigation of Huang Ren,or heat poisoning Yin,false fire and inflammation or trauma.By referring to ancient books and documents,analysis of the etiology and pathogenesis of common uveitis,it is found that the occurrence and development of uveitis is mostly related to liver.In the treatment,uvea disease can be treated from liver.
