摘要
【目的】观察荷叶饮治疗痛风性关节炎间歇期高尿酸血症湿热瘀阻型患者的临床疗效。【方法】将50例痛风性关节炎间歇期高尿酸血症湿热瘀阻型患者随机分为治疗组和对照组,每组各25例。对照组给予口服苯溴马隆片治疗,治疗组给予口服荷叶饮汤剂治疗,疗程为4周。观察2组患者治疗前后血尿酸水平和中医证候积分的变化情况,并评价2组患者的临床疗效和安全性。【结果】(1)疗效方面:治疗4周后,治疗组的总有效率为88.0%(22/25),对照组为68.0%(17/25),组间比较,治疗组的临床疗效明显优于对照组(P<0.05)。(2)血尿酸水平方面:治疗后,2组患者的血尿酸水平均较治疗前明显降低(P<0.05),且治疗组对血尿酸水平的降低作用明显优于对照组(P<0.05)。(3)中医证候积分方面:治疗后,治疗组患者的中医证候积分较治疗前明显降低(P<0.05),而对照组无明显降低(P>0.05),组间比较,治疗组对中医证候积分的降低作用明显优于对照组(P<0.01)。(4)安全性方面:治疗组的不良反应发生率为0(0/25),对照组为16.0%(4/25),组间比较,差异无统计学意义(P>0.05)。【结论】荷叶饮治疗痛风性关节炎间歇期高尿酸血症湿热瘀阻型患者疗效确切,可有效改善患者中医证候,降低患者血尿酸水平。
Objective To observe the clinical efficacy of Heye Decoction(a herbal compound prescription mainly with the actions of nourishing yin,clearing heat and arresting bleeding)on gouty arthritis(GA)patients with intermittent hyperuricemia of damp-heat obstruction type.Methods Fifty GA patients with intermittent hyperuricemia of damp-heat obstruction type were randomly divided into treatment group and control group,with25 cases in each group.The control group was given oral use of Benbromarone tablets and the treatment group was given oral use of Heye Decoction for 4 weeks.The changes of serum uric acid level and traditional Chinese medicine(TCM)syndrome scores in the two groups were observed before and after treatment.After treatment,the clinical efficacy and safety of the two groups were evaluated.Results(1)Clinical efficacy:after 4 weeks of treatment,the total effective rate of the treatment group was 88.0%(22/25)and that of the control group was 68.0%(17/25).The intergroup comparison showed that the clinical efficacy of the treatment group was significantly superior to that of the control group(P<0.05).(2)Blood uric acid level:after treatment,blood uric acid level in both groups was significantly decreased compared with that before treatment(P<0.05),and the effect on decreasing blood uric acid level of the treatment group was significantly superior to that of the control group(P<0.05).(3)TCM syndrome scores:after treatment,the TCM syndrome scores in the treatment group was significantly decreased compared with those before treatment(P<0.05),while those in the control group were not significantly decreased(P>0.05).The intergroup comparison showed that the effect on decreasing TCM syndrome scores in the treatment group was significantly superior to that in the control group(P<0.01).(4)Safety:the incidence of adverse reactions was 0.0%(0/25)in the treatment group and was 16.0%(4/25)in the control group,with no statistical difference between the two groups(P>0.05).Conclusion Heye Decoction exerts certain effect on gouty arthritis patients with intermittent hyperuricemia of damp-heat obstruction type,and the decotion can effectively improve the TCM syndromes of the patients and reduce the level of blood uric acid.
