摘要
目的:探究通肝生乳汤联合穴位按摩治疗产后泌乳不足(肝郁气滞型)的效果观察。方法:选取我院80例泌乳不足产妇为研究对象,按照简单随机法分为联合组和对照组各40例,对照组给予穴位按摩治疗,联合组在对照组基础上加以通肝生乳汤治疗,比较两组临床疗效、中医证候积分、干预1周内泌乳量变化情况、产后开始泌乳时间、产后48h母乳喂养率、产后30d纯母乳喂养率及血清激素[催乳素(PRL)、雌二醇(E2)、孕酮(P)]水平变化情况。结果:联合组临床有效率为96.67%,显著高于对照组的86.67%,差异有统计学意义(P<0.05);治疗7d后,两组中医症候积分均显著降低(P<0.05),且治疗后组间对比差异显著(P<0.05);与治疗前相比,治疗3d、5d、7d后两组乳汁分泌评分均依次显著降低(P<0.05),且治疗后各时间点组间对比差异显著(P<0.05);联合组产后开始泌乳时间显著低于对照组(P<0.05),产后48h母乳喂养率显著高于对照组(P<0.05),两组产后30d纯母乳喂养率比较差异无统计学意义(P>0.05);治疗7d后,两组血清PRL水平均显著升高(P<0.05),E2、P水平均显著降低(P<0.05),且治疗后组间对比差异显著(P<0.05)。结论:通肝生乳汤联合穴位按摩治疗产后母乳不足可显著提高临床疗效,加快症状改善,促进泌乳正常。
Objective:To explore the effect of using Tonggan Shengru Decoction combined with acupoint massage in the treatment of liver-Qi stagnation type postpartum hypogalactia.Methods:80 puerperae with hypogalactia in the hospital were selected as the research subjects,and were divided into a combined group(40 cases)and a controlled group(40 cases)according to the simple random method.The controlled group was given acupoint massage,and the combined group was additionally given Tonggan Shengru Decoction on the basis of the controlled group.The clinical efficacy,scores of TCM syndromes,changes in lactation yield within 1 week of intervention,postpartum lactation start time,breastfeeding rate at 48 h after delivery,exclusive breastfeeding rate at 30 d after delivery and levels of serum hormones[prolactin(PRL),estradiol(E),progesterone(P)]were compared between the two groups.Results:The clinical effective rate of 96.67%in combined group was significantly higher than 86.67%in the controlled group(P<0.05).After 7 d of treatment,the scores of TCM symptoms in both groups were significantly decreased(P<0.05),and there were significant differences between the groups after treatment(P<0.05).Compared with before treatment,the lactation scores of the two groups were reduced significantly after 3 d,5 d and 7 d of treatment(P<0.05),and there was a significant difference between the groups at each time point after treatment(P<0.05).The postpartum lactation start time in the combined group was significantly shorter than that in the controlled group(P<0.05),and the breastfeeding rate at 48 h after delivery was significantly higher than that in the controlled group(P<0.05),but there was no statistical significance between the two groups in terms of exclusive breastfeeding rate at 30 d after delivery(P>0.05).After 7 d of treatment,the levels of serum PRL in the two groups were significantly enhanced(P<0.05)while the levels of Eand P were significantly declined(P<0.05),and the differences between the groups were significant after treatment(P<0.05).Conclusion:Using Tonggan Shengru Decoction combined with acupoint massage in the treatment of postpartum hypogalactia can significantly improve the clinical efficacy,accelerate the improvement of symptoms,and promote the normal lactation.
