摘要
目的:探讨中药汤剂联合穴位埋线治疗慢性荨麻疹的临床疗效及安全性。方法:选取2020年3月~2021年3月榆林市中医医院收治的100例慢性荨麻疹患者,随机数字表法分为三组,对照组1(n=33)接受盐酸左西替利嗪片治疗,对照组2(n=33)接受加味麻辛附子汤联合盐酸左西替利嗪片治疗,试验组(n=34)接受加味麻辛附子汤联合穴位埋线治疗。比较三组患者临床疗效和治疗后7日荨麻疹活动度评分(UAS7)、慢性荨麻疹生活质量问卷(CU-Q2oL)、外周血T淋巴细胞水平和血清白介素-4(IL-4)、IL-10、IL-17、IL-23、IL-35水平及不良反应发生情况。结果:试验组临床治疗总有效率明显高于对照组1、对照组2,差异有统计学意义(P<0.05),对照组1与对照组2治疗总有效率比较无差异(P>0.05)。治疗后试验组UAS7评分、CU-Q2oL评分、外周血CD8^(+)百分比、血清IL-4、IL-10、IL-17、IL-23水平明显低于对照组1、对照组2,外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)和血清IL-35水平明显高于对照组1、对照组2(P<0.05),对照组1与对照组2比较无差异(P>0.05)。三组患者均未出现明显不良反应。结论:加味麻辛附子汤联合穴位埋线治疗慢性荨麻疹(风寒束表型)疗效确切,可显著改善患者皮损和瘙痒症状,提升生活质量,调节免疫功能和细胞因子水平,且安全性高,值得临床推广。
Objective:To explore the clinical efficacy and safety of applying traditional Chinese medicine decoction combined with acupoint catgut embedding in the treatment of chronic urticaria.Methods:100 patients with chronic urticaria in our hospital(March 2020 to March 2021)were divided into three groups according to the random digital table method.The controlled group 1(n=33)was treated with Levocetirizine Hydrochloride Tablets,the controlled group 2(n=33)received Modified Maxin Fuzi Decoction with Levocetirizine Hydrochloride Tablets.The treatment group(n=34)received Modified Maxin Fuzi Decoction with acupoint embedding therapy.Comparison of the clinical efficacy and after treatment urticaria activity score 7(UAS7),chronic urticaria quality of life questionnaire(CU-Q2 oL),peripheral blood T lymphocyte levels,and serum interleukin-4(IL-4),IL-10,IL-17,IL-23,IL-35 levels and the occurrence of adverse reactions in the three groups of patients.Results:The total effective rate of clinical treatment in the treatment group was significantly higher than that of the controlled group 1 and the controlled group 2(P<0.05),there was no difference in the total effective rate of treatment between the controlled group 1 and the controlled group 2(P>0.05).After treatment,the UAS7 score,CU-Q2 oL score,peripheral blood CD8^(+)percentage,serum IL-4,IL-10,IL-17,and IL-23 levels in the treatment group were significantly lower than those in the controlled group 1 and the controlled group 2,and the peripheral blood CD3^(+),CD4^(+)The levels of CD4^(+)/CD8^(+)and serum IL-35 were significantly higher than those of the controlled group 1 and the controlled group 2(P<0.05),and there was no difference between the controlled group 1 and the controlled group 2(P>0.05).There were no obvious adverse reactions in the three groups of patients.Conclusion:The combination of Modified Maxin Fuzi Decoction with acupoint embedding has a definite effect in the treatment of chronic urticaria,which can significantly improve the skin lesions and itching symptoms of patients,improve the quality of life,regulate immune function and cytokine levels,and it has high safety and is worthy of clinical promotion.
