摘要
目的探讨益气温阳汤联合枸地氯雷他定治疗老年慢性荨麻疹患者的临床疗效及对CD4^+、CD8^+、CD4^+/CD8^+水平的影响。方法选取2017年1月至2019年5月大连市皮肤病医院中医皮肤科门诊收治的慢性荨麻疹者患者68例作为研究对象,按随机数字表法分为观察组与对照组,各34例。对照组口服枸地氯雷他定片,观察组在口服枸地氯雷他定片基础上联合益气温阳汤治疗。比较两组患者临床疗效、临床症状积分、CD4^+、CD8^+、CD4^+/CD8^+水平变化及临床复发率。结果观察组治疗有效率为94.12%(32/34),对照组为76.47%(26/34),差异有统计学意义(P<0.05)。治疗后,两组患者临床症状积分均较治疗前明显下降,且观察组明显低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组血清CD4^+、CD4^+/CD8^+明显高于对照组,CD8^+明显低于对照组,差异有统计学意义(P<0.05)。随访3个月,观察组复发率为12.50%,对照组为34.62%,差异有统计学意义(P<0.05)。结论益气温阳汤联合枸地氯雷他定治疗老年慢性荨麻疹临床疗效显著,可改善患者临床症状,调节血清CD4^+、CD8^+、CD4^+/CD8^+表达水平,降低复发率。
Objective To explore the clinical effects of Yiqi Wenyang Decoction combined with medloratadine in the treatment of elderly patients with chronic urticaria and its influence on the levels of CD4^+,CD8^+,CD4^+/CD8^+.Methods From January 2017 to March 2019,68 patients with chronic urticaria admitted to the Dermatology Clinic of Traditional Chinese Medicine of Dalian Dermatology Hospital were selected as the study object.They were divided into the observation group and the control group according to the random number table method,with 34 cases in each group.The control group was treated with medeciloratadine,and the observation group was treated with Yiqi Wenyang Decoction on the basis of medeciloratadine.The clinical efficacy,clinical symptom score,CD4^+,CD8^+,CD4^+/CD8^+levels and clinical recurrence rate were compared between the 2 groups.Results The effective rate was 94.12%(32/34)in the observation group and 76.47%(26/34)in the control group,the difference was statistically significant(P<0.05).After treatment,the clinical symptom scores of the 2 groups were significantly lower than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the serum CD4^+and CD4^+/CD8^+in the observation group were significantly higher than those in the control group,and CD8^+was significantly lower than that in the control group(P<0.05).The recurrence rate was 12.50%in the observation group and 34.62%in the control group,the difference was statistically significant(P<0.05).Conclusion The clinical effects of Yiqi Wenyang Decoction combined with medloratadine in the treatment of senile chronic urticaria is significant.It can improve the clinical symptoms of the patients,regulate the expression levels of CD4^+,CD8^+,CD4^+/CD8^+,and reduce the recurrence rate.
