摘要
目的研究四川省攀枝花市2015—2019年韦太夫雷登沙门菌食源株、食物中毒暴发株、腹泻散发株的流行病学特征与分子分型特征。方法对16株菌株进行血清、脉冲场凝胶电泳(pulsed field gel electrophoresis,PFGE)和耐药表型分析。利用大数据,结合实验室检测结果、流行病学资料、临床资料对其流行和分子分型特征进行描述分析。结果16株沙门菌抗原式均为3,10∶r∶z6;16株菌株PFGE图谱分为5种带型,相识度系数81.7%~100.0%,菌株各个克隆系亲缘关系相近,有高度同源性。A簇为优势克隆簇;3株食品株中1株对四环素和复方磺胺耐药,对环丙沙星中介。3株腹泻株中1株对复方磺胺耐药,1株对四环素耐药,对环丙沙星中介。其余菌株对抗生素均敏感。文献显示,我国于1984年首次报道韦太夫雷登沙门菌。1984—2016年共报道其暴发事件19起。结论在我国华南地区流行的韦太夫雷登沙门菌已逐渐向内陆扩散。PFGE图谱显示,攀枝花市该菌的食品株与病例间有很近的亲缘关系。对15种抗生素几乎都敏感,未发现耐多药菌株,但仍需持续进行耐多药监测。
Objective Study the epidemiological characteristics and molecular typing characteristics of food source strains,food poisoning outbreak strains and diarrhea sporadic strains of Salmonella Weltevreden in Panzhihua,Sichuan from 2015 to 2019.Methods Sixteen strains were analyzed for theserum,pulsed field gel electrophoresis(PFGE)and drug resistance phenotype.The epidemic and corresponding molecular typing characteristics were described and analyzed by means of big data in combination with laboratory test results,epidemiological data and clinical data.Results The antigenic formula of 16 strains of Salmonella was 3,10∶r∶z6;the PFGE maps of 16 strains were divided into 5 types,and the coefficient of acquaintance ranged between 81.7%and 100.0%.Clones were found to have similar genetic relationship and be highly homologous.Cluster A was the dominant clone cluster.One of the three food strains was resistant to tetracycline and compound sulfonamide and was a ciprofloxacin mediator.In the three diarrhea strains,one was resistant to sulfanilamide,one was resistant to tetracycline,and the last one was moderate to ciprofloxacin.The remaining strains were susceptible to antibiotics,and literature showed that the first case of Salmonella Weltevreden in China was reported in 1984,and 19 outbreak events were reported between 1984 and 2016.Conclusion Salmonella Weltevreden prevalent in Southern China has gradually spread to the inland.PFGE maps show that food strains of Salmonella Weltevreden in Panzhihua are closely related to cases.Almost all of the food strains are susceptible to 15 kinds of antibiotics,no multi drug resistant strains are found,and multiple-drug resistance monitoring remains necessary.
