The present study aimed to compare their efficacy in end-stage hepatic alveolar echinococcosis patients who underwent ex-situ liver resection and autotransplantation (ELRA). Methods: A total of 81 patients received ELRA from August 2010 to March 2018. They were allocated into UW ( n = 48) and HTK groups ( n = 33) based on the type of solutions used. Demographic and operational data were retrospectively analyzed. Primary outcomes included 90-day mortality, incidence of early graft loss, primary dysfunction, and postoperative complications. Results: Demographic and operational characteristics were similarly distributed in the two groups. No statistically significant differences were observed with regard to 90-day mortality (12.77% vs. 12.12%) and early graft loss rate (8.51% vs. 9.09%) between the two groups. Patients in the UW and HTK groups showed a primary dysfunction rate of 27.66% and 27.27%, respectively. The UW group exhibited a higher incidence tendency of biliary complications, albeit with no statistical significance. Conclusions: This is the largest cohort study comparing the efficacy of the UW and HTK organ-preserving solutions in end-stage hepatic alveolar echinococcosis patients in ELRA settings. UW and HTK solutions presented similar efficacy and safety. A randomized clinical trial with larger scale is needed for further investigation in future clinical applications.展开更多

METHODS: Male Wistar rats were used as donors. Small bowels were retrieved, flushed and then stored in the respective 4 solutions for 18 h at 4℃. Functional integrity of the grafts was evaluated by isolated reperfusion with oxygenated Krebs-Henseleit buffer at 37℃ for 30 min in all 4 groups. RESULTS: Polysol preservation exhibited the highest tissue ATP concentration and the lowest release of LDH. Malondialdehyde, an index for tissue lipid peroxidation, was also the lowest in Polysol. Tissue oxygen consumption was significantly higher in Polysol than in the others. Of interest, UW-storage promoted 10-fold higher apoptosis than in the others. Moreover, electron microscopy revealed that the mucosal villi/micro-villi formation and the cell organelles, including mitochondria, were both significantly better preserved in Polysol, while deleterious alterations were apparent in the others, most notably in UW. Although Celsior and HTK exhibited the better trend of results than UW in some parameters, but could not reach the over-all superiority to UW.CONCLUSION: Cold storage using Polysol resulted in significantly better integrity and function of small bowel grafts than UW. Hence, Polysol may be a novel alternative for the small bowel preservation.展开更多

Liver proteolysis in tissue specimens and perfusate was measured by reverse-phase high performance liquid chromatography. Total free amino acid release was correlated with the activation of the ubiquitin proteasome system (UPS: measured as chymotryptic-like activity and 20S and 19S proteasome), the prevention of liver injury (transaminases), mitochondrial injury (confocal microscopy) and inflammation markers (TNF 1 alpha, high mobility group box-1 (HGMB-1) and PPAR gamma), and liver apoptosis (TUNEL assay, cytochrome c and caspase 3).RESULTSProfiles of free AA (alanine, proline, leucine, isoleucine, methionine, lysine, ornithine, and threonine, among others) were similar for tissue and reperfusion effluent. In all cases, the IGL-1 solution showed a significantly higher prevention of proteolysis than UW (P < 0.05) after cold ischemia reperfusion. Livers conserved in IGL-1 presented more effective prevention of ATP-breakdown and more inhibition of UPS activity (measured as chymotryptic-like activity). In addition, the prevention of liver proteolysis and UPS activation correlated with the prevention of liver injury (AST/ALT) and mitochondrial damage (revealed by confocal microscopy findings) as well as with the prevention of inflammatory markers (TNF1alpha and HMGB) after reperfusion. In addition, the liver grafts preserved in IGL-1 showed a significant decrease in liver apoptosis, as shown by TUNEL assay and the reduction of cytochrome c, caspase 3 and P62 levels.CONCLUSIONOur comparison of these two preservation solutions suggests that IGL-1 helps to prevent ATP breakdown more effectively than UW and subsequently achieves a higher UPS inhibition and reduced liver proteolysis.展开更多

OBJECTIVE: This study was designed to investigate the effects of asthma on the stability of WCST measurements through the use of a twin study, and to analyze whether egg-type difference exists. DESIGN: A cohort study. SETTING: Mental Health Center, the First Affiliated Hospital of Chongqing Medical University. PARTICIPANTS: Fifty-nine pairs of twins, aged 6-16 years, were primarily selected from schools between August 2005 and February 2007 and the WCST and Zygosity identification test was applied. Twins with achromatopsia, severe upper limb diseases, somatic diseases, or mental disorders were excluded. According to disease history of asthmatic attack, children were assigned into asthma (n = 16) and non-asthma (n = 43) groups. METHODS: Four WCST measurements were determined in the 59 pairs of twins, and egg-type differences were identified in conjunction. RESULTS: All 59 pairs of twins were included in the final analysis. Among the pairs of twins, 28 (48.5 %) were monozygotic twins, and 31 (52%) were dizygotic twins. Among the monozygotic and dizygotic twins, the number of preservative errors and percentage of preservative errors were significantly higher in the asthma group than in the non-asthma group (P 〈 0.05). However, there was no significant difference of each corresponding measure between the monozygotic and dizygotic twins (P 〉 0.05). CONCLUSION: Asthma may affect the stability of WCST measures, but egg-type differences do not exist.展开更多

目的:使用中文版威斯康星结石生活质量问卷(Chinese version of Wisconsin stone quality of life questionnaire,C-WISQOL)评估肾结石患者的健康相关生活质量(health related quality of life,HRQOL),预测导致患者HRQOL降低的危险因素...目的:使用中文版威斯康星结石生活质量问卷(Chinese version of Wisconsin stone quality of life questionnaire,C-WISQOL)评估肾结石患者的健康相关生活质量(health related quality of life,HRQOL),预测导致患者HRQOL降低的危险因素。方法:自2020年7月至2021年6月前瞻性纳入就诊于北京大学人民医院的肾结石患者,入组标准为18~80岁有足够汉语基础的肾结石患者,排除标准包括体内留置输尿管支架管的患者以及患有恶性肿瘤、脓毒症等的患者。收集患者的人口统计学资料和肾结石相关的临床资料,记录患者填写的C-WISQOL问卷和健康调查简表(short form 36 health survey,SF-36)。C-WISQOL包括四个维度(domain,D):情感影响(D1)、社会影响(D2)、结石相关影响(D3)和活力影响(D4)。使用Cronbach’sα系数验证C-WISQOL的内部一致性,使用Spearman’s rho系数验证C-WISQOL和SF-36之间的校标关联效度,使用验证性因子分析验证C-WISQOL的结构效度、聚合效度和区分效度,使用单因素和多因素分析探讨导致肾结石患者HRQOL下降的危险因素。结果:共纳入307例肾结石患者,其中212例(69.1%)为男性,平均年龄(51.9±13.5)岁,平均体重指数(body mass index,BMI)(25.4±3.6)kg/m^(2),160例(52.1%)患者合并代谢综合征(metabolic syndrome,MS),202例(65.8%)有既往结石病史,217例(70.7%)有结石相关症状,53例(17.3%)为双侧肾结石,82例(26.7%)合并输尿管结石,199例(64.8%)伴有肾积水,78例(25.4%)伴有肾功能不全,168例(54.7%)合并泌尿系感染(urinary tract infection,UTI),肾结石的平均直径为(15.6±5.9)mm。所有患者C-WISQOL问卷的平均总分为(94.9±13.7)分,D1为(27.2±4.2)分,D2为(23.8±3.7)分,D3为(27.0±3.6)分,D4为(10.1±1.9)分。C-WISQOL问卷总分的Cronbach’sα系数为0.968,四个维度为0.860~0.898。C-WISQOL和SF-36问卷总分相关性的Spearman’s rho系数为0.564,维度间相关性为0.684~0.901,表明C-WISQOL有较好的内部一致性和校标关联效度。验证性因子分析显示,C-WISQOL具有较好的结构效度、聚合效度和区分效度。单因素分析显示,合并MS(OR=1.607,P<0.001)、有结石相关症状(OR=1.268,P<0.001)、双侧肾结石(OR=1.900,P<0.001)、合并输尿管结石(OR=1.018,P<0.001)、伴有肾积水(OR=1.685,P<0.001)、伴有UTI(OR=1.275,P<0.001)是肾结石患者HRQOL降低的危险因素;多因素分析表明,合并MS(OR=1.475,P<0.001)、合并输尿管结石(OR=1.546,P=0.043)、伴有UTI(OR=1.646,P=0.005)是肾结石患者HRQOL下降的独立危险因素。t检验结果显示,无MS、结石相关症状和UTI患者的C-WISQOL得分均显著高于合并相关危险因素组(P<0.001)。结论:C-WISQOL问卷可用于评估肾结石患者的HRQOL,具有较好的可靠性。合并MS、结石相关症状和UTI是肾结石患者HRQOL降低的独立危险因素。展开更多
