摘要
目的 评价切割球囊血管成形术对血清肿瘤坏死因子 α(TNF α)和白细胞介素 6 (IL 6 )、白细胞介素 8(IL 8)浓度的影响 ,并研究术后血清TNF α、IL 6和IL 8的浓度与远期心血管事件的联系。方法 冠状动脉造影证实的冠心病病人共 80例 ,分为普通球囊加支架组 ( 4 3例 )和切割球囊加支架组 ( 37例 ) ,所有病人采用酶联免疫吸附法测定术前、术中、术后即刻、术后 6h、术后 2 4h的血清TNF α、IL 6和IL 8的浓度 ,并随访术后 6个月心血管事件 ,比较再狭窄组与无再狭窄组之间血清TNF α、IL 6和IL 8的差异。结果 再狭窄组中普通球囊加支架组和切割球囊加支架组术后 6hTNF α、IL 6和IL 8浓度均较术前明显升高 ,但普通球囊加支架组升高更明显 ,而术后 2 4h与术前差异无显著性 ;无再狭窄的普通球囊加支架组和切割球囊加支架组各时间段的TNF α、IL 6和IL 8浓度均无显著变化。结论 切割球囊加支架术后血管损伤较小、炎症反应较轻 ;冠状动脉血管成形术后 6hTNF α、IL 6和IL 8浓度升高可能是术后发生再狭窄的预报标志 。
Objective To evaluate the effect of cutting balloon angioplasty and stent implantation on serum TNF α, IL 6 and IL 8, and investigate the relationship between the concentrations of serum TNF α, IL 6 and IL 8 after operation and the incidences of late cardiac events after intervention Methods Eighty patients underwent PTCA with normal balloon or cutting balloon Values of serum TNF α, IL 6 and IL 8 before and after intervention were measured using enzyme linked immuno sorbent assay Results The concentrations of serum TNF α, IL 6 and IL 8 were increased significantly in the restenosis group, especially in the POBA group There was no obvious difference in no restenosis group Conclusion The lower restenosis rate in cutting balloon group was related to its lower injury and inflammation of blood vessels The concentrations of serum TNF α, IL 6 and IL 8 6 hours after intervention may be the predictable sign of restenosis after intervention
