摘要
目的:动态观察利陪酮对慢性精神分裂症阴性、阳性症状的疗效和评价副反应。方法:对74例慢性精神分裂症均因原来使用的抗精神药物疗效不理想而换用利培酮治疗,疗程20周。在用药前及用药后每月用BPRS、SAPS、SANS及TESS量表进行6次评分,观察治疗期阃各个量表总分的变化情况、治疗中各个时期的疗效及判定其总体疗效。用TESS量表观察其副反应情况。结果:①治疗前和治疗后的前3个月,BPRS、SAPS两个量表相邻前后两次评分均有显著性差异,而后三次评分中相邻两次评分间无明显差异;在SANS量表的6次评分中相邻前后两次评分均有显著性差异。②从治疗的各个时期的疗效看也表现出和相邻两次各量表平分差异性比较相一致的结果。③观察结束时与治疗前比较,各量表均显示很高的疗效。④最常见的副作用的是EPS(轻度)和失眠。结论:利培酮不仅对慢性精神分裂症阳性症状有较好疗效,对其阴性症状疗效更为显著,对阳性症状疗效随治疗时间增加而增加,当达到一定的疗效水平后疗效进一步提高变得不明显,而对阴性症状的疗效有持续性增高的趋势;副反应轻微,具有较好的安全性。
Objective:To observe the therapeutic effectiveness and side reaction of Risperidon for negative and positive symptom of chronic schizophrenia by dynamic state. Methods: Seventy-four chronic schizophrenias whose effectiveness was low with other anti - psychotic medicine were treated 20 weeks with Risperidon . The clinic symptoms were measured with BPRS, SAPS and SANS and the side reactions were measured with TESS in sixth every month before and after treatment, we scaled and analyzed results. Result: (1) Adjacent result of BPRS and SAPS have significant difference in after treatment during the first, second, third month and have no significant difference in late three scale after treatment. All adjacent result of SANS compare has significant difference in statistics. (2) Therapeutic effectiveness results of every term clinic observation compare with every times Adjacent result have high significant difference. (3) Therapeutic effectiveness result of scale has significant difference in before treatment and after treatment 20 weeks.(4) EPS and insomnia ware common. Conclusion: Risperidon has better effectiveness not only for positive and negative symptom of chronic schizophrenia but can persistent improve treatment action for negative symptom. Rispendon has low side reaction and safety coefficient .
