危重症中心静脉插管相关性感染致病菌分析及预防 被引量:21

Prevention and pathogenesis of intravascular catheter-related infections in critically ill patients

摘要 目的 分析中心静脉插管相关性感染的致病菌分布及相关危险因素 ,以指导临床的治疗及预防。方法  1997~ 2 0 0 1年对 41例危重症病人中心静脉导管远端培养阳性病人的临床资料进行回顾性分析。结果  41例导管培养阳性中常见致病菌有真菌 (2 1/ 4 1) ,G+ 球菌 (12 / 4 1)及G-菌 (8/ 4 1)。 41例中心静脉插管感染中 ,45 6%的病例发生在 60岁以上的病人 ;60 9%的病例发生在胃肠道功能不全需静脉营养的病人 ;70 8%的病例发生在中心静脉导管留置时间超过 2周 ;58 5%的病例血培养同样致病菌阳性。结论 中心静脉插管相关性感染的致病菌主要是真菌和葡萄球菌 ; ObjectiveThrough investigating the pathogens and risk factor s of intravascular catheter-related infections,try to find a better way for its treatment and prophylaxis.MethodsThe data of 41 critically ill patients with intravascular c atheter-related infections between 1997 and 2001 were reviewed.Results The majority of infection patho gens were yeast-like fungi (21/41) , Candida species(8/41) and staphylococcus spec ie s(12/41).Of all the 41 cases,45 6% were older than 60 years,60 9% occurred in patients needing total parenteral nutri ti on,70 8% were those with the duration of catheter insertion exceeding 2 wee ks; 58 5% had the same pathogens with blood cultur e .ConclusionThe most common pathogens of intravascular cathete r-related infec tions are fungi and staphylococcus species. To shorten the duration of catheter insertion and establish enteral nutrition earlier are helpful for the control o f intravascular catheter-related infections.
