可溶性Fas及其配体在小儿病毒性心肌炎中临床意义研究

摘要 目的 探讨可溶性Fas及其配体在小儿病毒性心肌炎中的表达以及与细胞凋亡和免疫反应的关系 ,旨在为病毒性心肌炎的诊断和治疗提供新的线索和依据。方法 采用ELISA法测定了 4 4例病毒性心肌炎患儿和 2 0例健康儿童血清中sFas和sFasL的表达水平 ,并进行前瞻性对比研究。结果 病毒性心肌炎患儿血清sFas和sFasL表达水平较健康儿童明显增高 (P <0 .0 1)。病毒性心肌炎患儿血清sFas与sFasL表达水平呈正相关性 (r=0 .6 5 0 9,P <0 .0 1)。儿童血清sFas和sFasL表达水平无性别差异 (P >0 .0 5 )。结论 ①血清sFas及sFasL表达增高与病毒性心肌炎疾病过程有关。②sFas表达增高可进一步证实病毒性心肌炎发病与免疫因素有关。③血清sFas表达增高可望作为T淋巴细胞激活的标志以及病毒性心肌炎诊断和治疗的重要依据。 Objective To investigate the expression of the soluble Fas(sFas) and Fas ligand(sFasL) in children with viral myocarditis(VMC), and the relationship with apoptosis and immunity, with the aim to provide evidence and basis for the diagnosis and treatment of VMC. Methods The serum concentrations of sFas and sFasL in 44 VMC and 20 healthy children were determined by ELISA and were analyzed prospectively. Results The expression levels of both sFas and sFasL in children with VMC were significantly higher than those of healthy children(P<0.01). There was positive correlation between sFas and sFasL expression levels in children with VMC( r=0.6509, P<0.01). but there was no significant difference in the expression levels of sFas and sFasL between sexes(P>0.05). Conclusion The increased serum expression levels of sFas and sFas are related to the process of VMC, which confirms the relationship between the onset of VMC and the immune factors and which is expected to be a sign of activated T lyphocyte and an important basis for the diagnosis and treatment of VMC.
