摘要
目的:探讨28例右心室原发良性肿瘤患者手术治疗的预后。方法:回顾性分析中国医学科学院阜外医院2007年1月至2017年11月行手术治疗的28例右心室原发良性肿瘤患者的临床资料,分析其预后状况。结果:28例患者中,男性20例,女性8例,平均年龄(36.6±21.1)岁。病理结果提示,10例为黏液瘤,弹力纤维瘤、血管瘤、横纹肌瘤各4例,脂肪瘤、纤维瘤、淋巴管肿瘤各2例。患者均在全身麻醉体外循环、心脏停跳下行肿瘤切除术,24例行肿瘤完全切除术,4例行肿瘤部分切除手术。随访1～10年(中位随访39.5个月)期间,术后10年的免复发率和生存率分别为83.8%和79.0%。行肿瘤部分切除手术的4例患者,1例为横纹肌瘤患者,术后6年复查示瘤体消失;1例为血管瘤患者,术后3次复查显示瘤体在逐渐缩小;另外2例分别为脂肪瘤和纤维瘤,瘤体大小无明显变化。结论:对于右心室原发良性肿瘤,外科手术切除治疗安全、有效,预后良好。
Objectives:To investigate the prognosis of 28 cases with primary benign right ventricular tumors post-surgical treatment.Methods:Clinical data from 28 patients with right ventricular primary benign cardiac tumors underwent cardiac surgery in our hospital from January 2007 to November 2017 were retrospectively reviewed.We analyzed the outcome up to 10 years post operation.Results:There were 20 male patients and 8 female patients in this cohort and the mean age was 36.6±21.1 years-old.The postoperative pathological biopsy results were as follows:myxoma(10 cases),fibroelastoma(4 cases),hemangioma(4 cases),rhabdomyoma(4 cases),lipomas(2 cases),fibroma(2 cases),lymphangioma(2 cases).All patients underwent tumor resection surgery under general anesthesia and cardiopulmonary bypass followed by cardiac arrest.The median follow-up time was 39.5 months,and the recurrence-free rate and survival rate were 83.8%and 79.0%respectively.Four patients received partial resection of the tumor.For 1 patient with rhabdomyosarcoma,the tumor disappeared after 6 years of operation,and for 1 patient with hemangiomas,the tumor was gradually shrinking during the three times review after surgery.The size of the tumor remained unchanged for the rest 2 patients.Conclusions:Cardiac surgery is a safe and effective method and is linked with satisfactory prognosis for patients with primary benign right ventricular tumors.
