摘要
目的 观察血浆同型半胱氨酸 (Hcy)及其代谢相关因子与老年冠心病 (CHD)及糖尿病之间的关系。方法 选择老年冠心病患者 78例 ,其中伴发Ⅱ型糖尿病的 17例为A组 ;无伴发Ⅱ型糖尿病 6 1例为B组 ;选择 2 3例健康老年人作为对照组C组。对受试者进行血浆Hcy、叶酸和维生素B12 定量测定。结果 患病组老年血浆Hcy值明显高于健康老年组血浆Hcy值 ,血浆VitB12 、叶酸值明显低于健康对照组 ,两组间比较有统计学意义 (P <0 .0 5 ) ,Ⅱ型糖尿病组 (A组 )血浆Hcy值明显高于无糖尿病组 (B组 ) (P <0 .0 5 ) ,且A组VitB12 和叶酸值明显低于B组 (P <0 .0 1) ,B组血浆Hcy值高于C组 ,VitB12 明显低于C组 (P <0 .0 1) ,而叶酸两组间差别无统计学意义。患病组的Hcy值和叶酸、VitB12 呈明显的负相关 ,相关系数分别为 - 0 .6 1和 - 0 .4 9,P均 <0 .0 1。结论 老年冠心病患者高Hcy血症与动脉硬化、冠心病的发病及糖尿病胰岛素相对或绝对缺乏有密切关系 。
Objective To observe the relationship for plasm homocysteine(Hcy) and its metabolism correlative factor with coronary heart disease and diabetes.Methods In the elderly.The 78 patients were diagnosed as coronary heart disease male 56 cases,female 22 cases,from 64 to 88 years old,mean (72±5) years old,These patients were divided into suffering from noninsulin-dependent diabeter(NIDDM) group(group A,17 cases),not suffering from NIDDM group(group B 61 cases).At the same time ,23 health persons.Who were chosen as the control group(group C),male 17 cases,female 6 cases,from 59 to 78 years old,mean ( 69±8) years old,All of the objects were determined of their plasm Hcy,folec acid and Vit B 12.Results Level of plasm Hcy in the patient group was much higher than that of group C,level of plasm folec acid and Vit B 12 in the patient group was much lower than that of group C (P< 0.05),level of plasm Hcy in the group A was much higher than that of group B,Level of plasm folec acid and Vit B 12 in the group A was much lower than that of group B (P< 0.05),Level of plasm Hcy in the group B was much higher than that of in group C,Level of plasm Vit B 12 in the group B was much lower than that of group C (P< 0.05),there were no significant difference about the level of folec acid between group B and C.There was significant negative correlation between Hcy and folec acid and Vit B 12 in pateint group A and group B.Conclusion Hyperhomocysteinemia caused by insufficiency of folate and Vit B 12 as closely related with arterisclersis,CHD,diabetes in aged patients.The metabolism law about the Hcy has an important effect on prevention and cure of cardiovascular and cerebrovacular disease.
