摘要
目的:探讨腹腔镜与开放肾癌根治术对机体应激反应的影响。方法:66例肾癌患者随机分为观察组(n=33)与对照组(n=33),观察组给予腹腔镜下肾癌根治术,对照组给予开放肾癌根治术,对比两组患者机体应激反应情况。结果:两组患者术前血清皮质醇(COR)、心率、血压、白细胞计数(WBC)、C反应蛋白(CRP)和甲状腺素(FT3和FT4)比较均差异无统计学意义(P>0.05),术后第1天和第3天两组血清COR、WBC、CRP、心率和血压均显著增高(P<0.05),但观察组显著低于对照组(P<0.05);两组FT3和FT4在术后第1天和第3天均显著降低,但观察组显著高于对照组(P<0.05)。结论:腹腔镜肾癌根治术与开放肾癌根治术均可使机体产生一系列应激反应,但腹腔镜肾癌根治术机体应激反应小,患者恢复快,值得推广。
Objective:To discuss the clinical influence of on stress reaction of laparoscopic and open radical nephrectomy for renal carcinoma.Methods:66patients with renal carcinoma were randomly divided into the observation group(n=33)and the control group(n=33)The observation group were treated with laparoscopic radical nephrectomy and the control group were treated with open radical nephrectomy.The stress reaction was contrasted between the two groups.Results:There was no significantly difference between two groups in serum cortisol,heart rates,blood pressure,WBC,CRP,FT3 and FT4preoperation(P >0.05).Serum cortisol,heart rate,blood pressure,WBC and CRP in the two groups were significantly reduced 1and 3days postoperation(P<0.05)while the observation group was significantly higher than the control group(P<0.05);FT3and FT4 in the two groups were significantly increased 1and 3days postoperation(P<0.05)while the observation group was significantly lower than the control group(P<0.05).Conclusions:Laparoscopic and open radical nephrectomy can produce a series of stress reaction,but laparoscopic radical nephrectomy for renal carcinoma has a low stress reaction and fast recovery.It is worthy of promotion.
