摘要
目的 :通过血清E2 、FSH、LH测定及子宫和卵巢形态改变关系的研究 ,探讨特发性中枢性性早熟女孩的特点。方法 :分析了 6 7例特发性中枢性性早熟女孩血清E2 、FSH、LH的变化及子宫和卵巢B超形态学改变 ,并进行直线相关分析。结果 :(1)不同型卵巢的血清LH和E2 值差异有显著性 (p <0 0 1)且呈递增趋势。FSH在三组间无规律性改变。 (2 )子宫大小随成熟度增高而显著增大 ,子宫容积与血清LH、E2 之间显著相关 (p <0 0 0 2 ,0 0 0 1)。 (3)卵巢容积随发育分期增大 ,容积与血清LH、FSH、E2 均不相关。结论 :(1)血清LH和E2 的水平与最大卵泡直径相关 ,而与总容积不相关 ,但最大卵泡直径与LH、E2 及卵巢总容积显著相关 ,从而证实了LH与卵泡发育的关系及E2 的水平主要取决于卵泡发育状态。 (2 )子宫体容积与LH、E2 均相关 ,子宫的发育呈显著的雌激素依赖性。E2 与子宫发育间有密切关系。 (3)血清E2 、LH。
Objective To explore the characteristics of the girls with idiopathic central precocious puberty (ICPP) through study on the relatiouship between serum E 2, FSH, LH Levels and morphologic changes of uterus and ovary. Methods Serum E 2, FSH and LH levels were measured with ECLIA and morphologic changes of uterus and ovary were studied with B-US in 67 girls with idiopathic central precocious puberty. Results ①Among the three types of ovaries (type 1, homogeneous type; type 2, small follicalax type; type 3, large follicular type), differences of the serum LH and E 2 values were significant ( p <0 01) between type3 and type1,2 respectively; however, differences of the FSH levels were not significant. ②The size of uterus was getting bigger as the digree of maturation proceeded. The volume of uterus was significantly correlated to the serum LH, E 2 levels ( p<0 002,p<0 001 ). ③The volume of ovary expanded with the grade of development but was not correlated to the serum hormone levels. Conclusion ①Serum LH and E 2 levels were correlated to the maximal follicular diameter but not to the total volume of ovary. It proved that there was close relationship between LH levels and follicular development. It also proved that serum E 2 levels was mainly decided by the development status of the follicles irrespective the total ovary volume. ②Volume of uterus was correated to both LH and E 2 levels. Development of uterus was significantly estrogen-dependent. ③Judgement based upon observations on measurement of serum E 2, LH, FSH levels and morphologic changes of uterus and ovary would be crucial for estabilshing the diagnosis of idiopathic central precocious puberty.
