摘要
目的 :探讨纤维支气管镜 (纤支镜 )支气管肺段冲洗与机械通气联合救治外科术后肺部感染合并急性呼吸衰竭患者的疗效。方法 :对 6 6例外科术后肺部感染合并急性呼吸衰竭的患者随机分为两组 ,治疗组 (33例 )采用纤支镜支气管肺段冲洗与机械通气联合治疗 ,对照组 (33例 )采用机械通气治疗。观察两组治疗后动脉血气分析结果的变化、机械通气时间、平均住院时间、平均住ICU时间、一次拔管成功率、病死率、并发症发生率、器官衰竭发生率和术后总治疗费用的情况。结果 :治疗后治疗组与对照组相比 ,动脉血气分析中PH ,PaO2 ,PaCO2 ,SaO2指标明显好转 ,机械通气时间、平均住院时间、平均住ICU时间明显缩短 ,术后总治疗费用明显减少 ,差异均有显著性 (P <0 .0 5 ) ;一次拔管成功率、病死率、并发症发生率、器官衰竭发生率均优于对照组 ,但差异无显著性 (P >0 .0 5 )。结论 :纤支镜与机械通气联合应用救治外科术后肺部感染合并急性呼吸衰竭疗效优于单用机械通气 ,并可以使机械通气并发症减少 ,改善围手术期的预后。
Objective : To explore the effect of bronchoscope bronchial lavage joint mechanical ventilation to save the patients with surgical postoperative infection of the lung merge acute respiratory failure. Methods: To 66 patients with surgical postoperative infection of the lung merge acute respiratory failure were divided into two groups. The treatment group(n=33)acceted bronchoscope bronchial lavage joint mechanical ventilation.The controlled group(n=33) were treated with mechanical ventilation. Following treatment, Compare with two groups parameters of arterial blood gas(PH,PaO 2,PaCO 2,SaO 2) ,the times of mechanical ventilation ,average in hospital, average in ICU, success rate one times pull out windpipe conduit,death rate, complications happen rate , organ failure happen rate and all postoperative treatment costs. Results: Following treatment, compare with the controlled group, the treatment group patients parameters of arterial blood gas(PH,PaO 2,PaCO 2,SaO 2) had been improved significantly, the times of mechanical ventilation ,average in hospital, average in ICU had been cut short significantly,and all postoperative treatment costs decreased significantly , all showed a stastical differenc(P<0.05). Success rate one times pull out windpipe conduit,ill death rate, complications happen rate , and organ failure happen rate were better than the controlled group,but there were not showed a stastical differenc(P>0.05). Conclusion: The effect of bronchoscope bronchial lavage joint mechanical ventilation to save the patients with surgical postoperative infection of the lung merge acute respiratory failure was better than only mechanical ventilation.,and the mechanical ventilation complications could be decreased,and improve the prognosis around date of operative.
