血清同型半胱氨酸与2型糖尿病周围神经病变关系临床研究 被引量:5

Research on the relationship between the serum homocysteine and type 2 diabetes peripheral neuropathy

摘要 目的探讨血清同型半胱氨酸(Hcy)与2型糖尿病(T2DM)周围神经病变的关系,同时分析高Hcy是否是T2DM周围神经病变的独立危险因素之一。方法收集2012年10月至2013年12月在呼伦贝尔市人民医院内分泌科住院的60例T2DM,分为伴周围神经病变(DPN^+)组和不伴周围神经病变(DPN^-)组,两组各30例,用荧光偏振免疫检测法测定空腹Hcy水平。比较两组Hcy水平及高Hcy的发生率,同时对这些病人进行双下肢肌电图检查,通过运动、感觉神经潜伏期、波幅及速度的变化来探讨Hcy与DPN的关系。应用SPSS 13.0统计软件进行统计分析。结果 DPN^+组的空腹血清Hcy水平明显高于DPN^-组[(15.89±6.22)μmol/L对(12.90±2.77)μmol/L,P=0.02];DPN^+组中高同型半胱氨酸(Hcy>16.00μmol/L)的发生率明显高于DPN^-组(43.33%对13.33%,P=0.02)。相关性分析发现,血清同型半胱氨酸与所测腓总神经、胫神经、腓浅神经之间没有相关性。Logistic回归分析显示,高Hcy、年龄、高血压、血脂紊乱进入回归方程。结论高Hcy可能是DPN的独立危险因素之一,监测Hcy水平,或许可预测DPN的发生。 Objective Explored the relationship between the serum homocysteine and type 2 diabetes peripheral neuropathy, and to analyze whether homocysteine was one of the independent risk factors for type 2 diabetes peripheral neuropathy at the same time. Methods Collected 80 patients who hospitalized the endocrine department in our hospital from October 2012 to December 2013, according to inclusion and exclusion criteria to choose 60 patients, they were divided into type 2 diabetes with peripheral neuropathy (DPN^+) group and type 2 diabetes without peripheral neuropathy (DPN^-) group, and each group was 30. Fasting serum homocysteine (Hcy) was measured by the fluorescence polarization immunoassay method. Compared the levels of homocysteine between the two groups and the incidence of high homocysteine in the two groups, at the same time, examed the electromyography of these patients two lower limbs, through the changes of the motor and sensory nerve latency, amplitude and speed to explore the relationship between homocysteine and type 2 diabetes peripheral neuropathy. Used SPSS13.0 statistical software for statistical analysis. Results The level of fasting serum homocysteine in the DPN^+ group was significantly higher than that of DPN^-group [(15.89± 6.22) μmol/L vs.(12.90± 2.77) μmol/L, P=0.02). The incidence of high homocysteine in the DPN^+ group (Hcy>16.00 μmol/L) was significantly higher than that in the DPN^-group (43.33% vs. 13.33%, P=0.02). Correlation analysis found that serum homocysteine had had no correlation with the measured peroneus nerves,tibialis nerves or peroneus superfic nerves. Logistic regression analysis showed that high homocysteine, age, hypertension, dyslipidemia, entered the regression equation. Conclution The high level of the homocysteine might be one of the independent risks for type 2 diabetes peripheral neuropathy, and maybe monitoring the level of serum homocysteine predicted the occurrence of diabetic peripheral neuropathy.
