摘要
目的超声乳化白内障摘出术可导致泪膜功能异常,甚至干眼症,尤其是糖尿病患者更为明显。本研究旨在探讨合并糖尿病对白内障患者超声乳化手术前后眼表状态影响。方法选取2017-03-15-2018-06-15新乡医学院第三附属医院眼科就诊的白内障患者82例(120只眼)作为研究对象,确诊其糖尿病发生情况,将未合并糖尿病的患者作为A组,合并糖尿病的患者作为B组,比较两组患者手术前后干眼症状评分、角膜荧光素染色、泪膜破裂时间和基础泪液分泌试验的情况。结果治疗后,B组患者术后干眼症状评分为(1.11±0.78)分,高于A组的(0.65±0.14)分,t=4.834,P<0.001;B组角膜荧光素染色(fluorescent,FL)评分为(4.16±1.95)分,明显高于A组的(3.03±1.54)分,t=3.544,P<0.001;治疗后B组,泪膜破裂时间(break-up time,BUT)为(5.75±1.38)s,显著短于A组的(8.25±1.8)s,t=8.239,P<0.001;基础泪液分泌试验(schirmer I test,SIT)为(8.13±3.59)mm/5min,明显短于A组(13.16±3.10)mm/5min,t=8.201,P<0.001。结论糖尿病可加重白内障手术患者术后干眼症状和进一步破坏其泪膜完整性和功能,故对于合并糖尿病的白内障患者行超声乳化手术时,应密切观察此类患者的干眼症状,做好预防工作,提高患者术后恢复率。
OBJECTIVE Phacoemulsification cataract extraction can lead to abnormal tear film function,even dry eye,especially in diabetic patients.The purpose of this study is to investigate the effect of diabetes mellitus on ocular surface status of cataract patients before and after phacoemulsification.METHODS A total of 82 patients(120 eyes)with cataract treated in ophthalmology department of the Third Affiliated Hospital of Xinxiang Medical College from March15,2017 to June 15,2018 were selected as the study subjects.The occurrence of diabetes mellitus was confirmed.The patients without diabetes mellitus were treated as group A and the patients with diabetes mellitus were treated as group B.The dry eye symptom score,corneal fluorescein staining,tear film rupture time and basic tear secretion test were compared between the two groups before and after operation.RESULTS After treatment,the postoperative dry eye symptom score of group B was(1.11±0.78),higher than that of group A(0.65±0.14),t=4.834,P<0.001.The group B corneal fluorescein staining(FL)score was(4.16±1.95),which was significantly higher than that of group A(3.03±1.54),t=3.544,P<0.001.After treatment,the tear-rupture time(BUT)of group B was(5.75±1.38)s,which was significantly shorter than that of group A(8.25±1.8)s,t=8.239,P<0.001.The basic tear secretion test(schirmer I test,SIT)was(8.13±3.59)mm/5 min,significantly shorter than group A(13.16±3.10)mm/5 min,t=8.201,P<0.001.CONCLUSIONS Diabetes can aggravate postoperative dry eye symptoms and further destroy the tear film integrity and function in patients with cataract surgery.Therefore,when phacoemulsification is performed in cataract patients with diabetes,in order to further improve the postoperative recovery rate,it should closely observe their dry eye symptoms and do a good job of prevention.
