摘要
目的探讨静脉窦插管溶栓加导丝疏通治疗颅内静脉窦闭塞性颅内压增高的治疗效果。方法经上矢状窦插入导管鞘,滴注尿激酶溶栓的同时,在DSA下用导丝机械疏通上矢状窦、左右横窦。结果12例病人颅内压增高症状全部缓解,无1例死亡,无并发症发生。有7例视力逐渐恢复,1例术前失明术后未能复明。结论在DSA下进行静脉窦插管溶栓加导丝疏通,是治疗颅内静脉窦闭塞性颅内压增高的一个安全有效、创伤较小的方法。
Objective To explore the curative effect of fibrinolysis and guidewires dredging the venous sinus on intracranial hypertension produced by venous sinus thrombosis.Methods The superior sagittal sinus was catheterized.Urokinase was infused into the sagittal sinus and sigmoid sinus,where there was thrombosis.Moreover these occluded sinuses were gently dredged by guidwires under DSA monitoring in the patients with intracranial hypertension due to venous sinus thrombosis.Results The symptoms of intracranial hypertension were completly relieved in the12patients with venous sinus thrombosis,of whom,none died.There were not any postoperative complications.Of8patients with sight disturbance before the treatment,7gradully recovered from sight disturbance and1not after the treatment.Conclusions The fibrinolysis combined with dredging with guidewires is a safe and effective method under DSA monitoring for treating the patients with intracranial hypertesion due to venous sinuses thrombosis.
