摘要
目的了解中国热带地区(海南岛)癫痫的病因构成。方法回顾性纳入2007年1月至2015年12月在海南医学院附属医院确诊为癫痫的839例患者。收集患者人口学特征、现病史、症状、脑电图(EEG)、脑部核磁共振(MRI)或CT等影像资料、脑脊液检查、血液检查、既往手术史和家族史。采用国际抗癫痫联盟(ILAE)2010年修订的癫痫术语进行癫痫的病因分类。结果 839例癫痫患者发病年龄5周~92岁,平均(35±25.19岁);男性518例,女性321例(男:女=1.61:1.00),其中遗传性癫痫占8.34%,结构-代谢异常性癫痫占66.63%(其中脑卒中21.22%,脑外伤10.40%,颅内感染8.82%,肿瘤8.70%,皮质发育异常7.51%),原因不明性癫痫占25.03%。结论中国热带地区(海南岛)癫痫患者以结构-代谢性病变为主要病因,癫痫临床诊断中尚需加强病因筛查;ILAE新的病因分类有利于指导医师进行癫痫临床诊断。
Objective To investigate the primary causes of epilepsy in the tropical region of China(Hainan Island). Methods A total of 839 patients with epilepsy in the Affiliated Hospital of Hainan Medical University from Jan.2007 to Dec. 2015 were enrolled in the study. The clinical data of patients were collected, including demographic characteristics, present illness, semiology, EEG, cerebral CT or MRI, cerebrospinal fluid examination, blood tests, surgical and family history. Etiologies of epilepsy were categorized as the revised terminology suggested by the International League Against Epilepsy(ILAE) in 2010. Results The 839 patients were 5 weeks to 92 years old, with the median of 35 years,including 518 males and 321 females, with the sex ratio of 1.61:1.00. There were 8.34% of genetic epilepsies, 66.63%of structural-metabolic epilepsies(stroke 21.22%, trauma 10.40%, infection 8.82%, tumor 8.70%, malformations of cortical development 7.51%), and 25.03% of epilepsies with unknown causes. Conclusion Structural-metabolic epilepsies were the dominant causes in the tropical region of China(Hainan Island). Establishment of standard diagnostic criteria by implementing the new guidelines proposed by the ILAE would contribute to the clinical diagnosis of epilepsies.
