摘要
目的观察口炎清颗粒联合复方氯己定含漱液治疗牙龈炎的疗效,并探讨其对患者牙周指标、龈沟液肿瘤坏死因子-α(TNF-α)、可溶性细胞间黏附因子-1(sICAM-1)、一氧化碳(NO)水平的影响。方法选取2018年7月至2023年7月郑州市口腔医院收治的106例牙龈炎患者作为研究对象,按随机数表法为观察组和对照组各53例。对照组患者采用复方氯己定含漱液治疗,观察组患者在对照组治疗的基础上联合口炎清颗粒治疗,两组患者均持续治疗1个月,并随访6个月。治疗1个月后比较两组患者的临床疗效,以及治疗前后的龈沟出血指数(SBI)、牙龈指数(GI)、菌斑指数(PLI)和龈沟液TNF-α、sICAM-1、NO水平;比较两组患者治疗期间的不良反应发生情况;随访6个月后,比较两组患者的复发率。结果治疗1个月后,观察组患者的疗效总有效率为90.57%,明显高于对照组的73.58%,差异有统计学意义(P<0.05)。治疗前,两组患者的SBI、GI、PLI比较差异均无统计学意义(P>0.05);治疗1个月后,观察组患者的SBI、GI、PLI分别为(0.79±0.21)分、(0.63±0.15)分、(1.18±0.26)分,明显低于对照组的(0.98±0.25)分、(0.82±0.18)分、(1.44±0.34)分,差异均有统计学意义(P<0.05)。治疗前,两组患者的龈沟液TNF-α、sICAM-1、NO水平比较差异均无统计学意义(P>0.05);治疗1个月后,观察组龈沟液TNF-α、sICAM-1、NO水平分别为(12.15±2.37)pg/mL、(120.42±11.58)pg/mL、(2.21±0.67)μmol/mL,明显低于对照组的(16.05±2.51)pg/mL、(134.07±13.50)pg/mL、(2.86±0.75)μmol/mL,差异均有统计学意义(P<0.05)。治疗期间的观察组和对照组患者不良反应总发生率分别为5.66%、3.77%,差异无统计学意义(P>0.05)。随访6个月后,观察组中有2例患者失访,对照组有3例患者失访,观察组患者的复发率为1.96%,明显低于对照组的12.00%,差异有统计学意义(P<0.05)。结论口炎清颗粒联合复方氯己定含漱液治疗牙龈炎的疗效较好,其有助于改善患者牙周指标,降低龈沟液TNF-α、sICAM-1、NO水平,具有临床应用价值。
Objective To observe the efficacy of Kouyanqing Granules combined with Compound Chlorhexi-dine Gargle in the treatment of gingivitis and explore its effects on periodontal indices and the levels of tumor necrosis factor-α(TNF-α),soluble intercellular adhesion molecule-1(sICAM-1),and nitric oxide(NO)in gingival crevicular fluid.Methods A total of 106 patients with gingivitis admitted to Zhengzhou Stomatological Hospital from July 2018 to July 2023 were selected as the study subjects and randomly divided into an observation group and a control group,each with 53 cases.Patients in the control group were treated with Compound Chlorhexidine Gargle,while those in the observation group received Kouyanqing Granules in addition to the control group's treatment.Both groups of patients were treated for 1 month and followed up for 6 months.After 1 month of treatment,the clinical efficacy,sulcus bleeding index(SBI),gingival index(GI),plaque index(PLI),and levels of TNF-α,sICAM-1,and NO in gingival crevicular flu-id were compared between the two groups before and after treatment.Adverse reactions during treatment were also compared.After 6 months of follow-up,the recurrence rates of the two groups were assessed.Results After 1 month of treatment,the total effective rate in the observation group was 90.57%,significantly higher than 73.58%in the con-trol group(P<0.05).Before treatment,there were no significant differences in SBI,GI,or PLI between the two groups(P>0.05).After 1 month of treatment,the SBI,GI,and PLI in the observation group were(0.79±0.21)points,(0.63±0.15)points,and(1.18±0.26)points,respectively,significantly lower than(0.98±0.25)points,(0.82±0.18)points,and(1.44±0.34)points in the control group(P<0.05).Before treatment,there were no significant differences in TNF-α,sI-CAM-1,or NO levels in gingival crevicular fluid between the two groups(P>0.05).After 1 month of treatment,the levels of TNF-α,sICAM-1,and NO in the observation group were(12.15±2.37)pg/mL,(120.42±11.58)pg/mL,and(2.21±0.67)μmol/mL,respectively,significantly lower than(16.05±2.51)pg/mL,(134.07±13.50)pg/mL,and(2.86±0.75)μmol/mL in the control group(P<0.05).The total incidence of adverse reactions during treatment was 5.66%in the observation group and 3.77%in the control group,with no significant difference(P>0.05).After 6 months of fol-low-up,2 patients in the observation group and 3 in the control group were lost to follow-up.The recurrence rate in the observation group was 1.96%,significantly lower than 12.00%in the control group(P<0.05).Conclusion The combi-nation of Kouyanqing Granules and Compound Chlorhexidine Gargle demonstrates good efficacy in treating gingivitis,improves periodontal indices,and reduces TNF-α,sICAM-1,and NO levels in gingival crevicular fluid,demonstrating clinical value.
