摘要
目的解析脑CT灌注(CT perfusion,CTP)与头颈部CT血管造影(CT angiography,CTA)联合应用在脑卒中侧支循环评估中的诊断价值。方法选择2023年1—12月曹县人民医院收治的50例脑卒中患者,均行CTP与CTA检查。以数字减影血管造影(digital subtraction angiography,DSA)为基准,评估CTP、CTA及其联合应用在脑卒中侧支循环诊断中的效能与价值。结果DSA结果显示不良19例,良好31例;脑CTP结果显示不良17例,良好33例;头颈CTA结果显示不良15例,良好35例;联合检查结果显示不良18例,良好32例。脑CTP联合头颈CTA检查与单独脑CTP、头颈CTA检查的准确度比较,差异有统计学意义(P<0.05)。脑CTP联合头颈CTA检查与单独脑CTP、头颈CTA检查的灵敏度与特异度比较,差异均无统计学意义(P均>0.05)。灵敏度与特异度比较,差异均无统计学意义(P均>0.05)。结论联合使用脑CTP和头颈CTA检查可以更全面地评估脑卒中患者的侧支循环情况,有助于更准确地制定治疗方案并预测患者的预后结果。
Objective To analyze the value of cerebral CT perfusion(CTP)combined with head and neck CT angiography(CTA)on the diagnosis of collateral circulation after stroke.Methods A total of 50 stroke patients admitted to Caoxian People's Hospital from January to December 2023 were selected for CTP and CTA examination.Based on digital subtraction angiography(DSA),the efficacy and value of CTP,CTA and their combined application in the diagnosis of collateral circulation in stroke were evaluated.Results DSA results showed 19 cases of poor performance and 31 cases of good performance.The brain CTP results showed 17 cases of poor performance and 33 cases of good performance.The head and neck CTA results showed 15 cases of poor performance and 35 cases of good performance.The joint examination results showed 18 cases of poor performance and 32 cases of good performance.The accuracy of brain CTP combined with head and neck CTA examination was compared with that of brain CTP and head and neck CTA examination alone,the difference was statistically significant(P<0.05).There were no significant differences in sensitivity and specificity between brain CTP combined with head and neck CTA and single brain CTP or head and neck CTA(both P>0.05).Conclusion The combined use of cerebral CTP and head and neck CTA can more comprehensively evaluate the collateral circulation of stroke patients,and help to more accurately formulate treatment plan and predict the prognosis of patients.
