摘要
目的探讨对比氟-18标记的前列腺特异性膜抗原抑制剂正电子发射断层显像/X线计算机体层成像(^(18)F-PSMA-1007 PET/CT)与氟-18标记的脱氧葡萄糖正电子发射断层显像/X线计算机体层成像(^(18)F-FDG PET/CT)及MRI在前列腺癌(PCa)诊断及分期中的效能。方法选取本院疑似PCa的103例患者资料行回顾性分析,根据病理诊断为恶性76例,良性27例。以手术病理检查为金标准,比较^(18)F-PSMA-1007 PET/CT与^(18)F-FDG PET/CT及MRI诊断PCa的灵敏度、特异度与准确性,并分析其诊断PCa分期的效能。结果以病理检查结果为金标准,^(18)F-PSMA-1007 PET/CT诊断PCa敏感度为94.74%(72/76),特异度为100%(27/27);^(18)F-FDG PET/CT诊断PCa的敏感度为89.47%(68/76),特异度为70.37%(19/27);MRI诊断PCa的敏感度为84.21%(64/76),特异度为62.96%(17/27)。^(18)F-PSMA-1007 PET/CT、^(18)F-FDG PET/CT、MRI诊断PCa Kappa值分别为0.904、0.598、0.461。^(18)F-PSMA-1007 PET/CT诊断PCa符合率高于^(18)F-FDG PET/CT诊断与MRI诊断,误诊率低于^(18)F-FDG PET/CT诊断与MRI诊断,漏检率低于MRI诊断,差异有统计学意义(P<0.05)。^(18)F-PSMA-1007 PET/CT诊断包膜外侵犯、精囊腺侵犯、淋巴结转移、骨转移高于^(18)F-FDG PET/CT诊断与MRI诊断,差异有统计学意义(P<0.05)。结论^(18)F-PSMA-1007 PET/CT与^(18)F-FDG PET/CT及MRI诊断PCa诊断及分期均具有一定价值,但^(18)F-PSMA-1007 PET/CT诊断PCa与诊断分期的准确性更好。
Objective To investigate the comparison of fluorine-18-labeled prostate-specific membrane antigen inhibitor positron emission tomography/X-ray computed tomography(^(18)F-PSMA-1007 PET/CT)with fluorine-18-labeled deoxyglucose positron emission tomography/X-ray computed tomography(^(18)F-FDG PET/CT)and multiparametric magnetic resonance(MRI)in the diagnosis and staging.Methods A retrospective analysis was conducted on the data of 103 suspected prostate cancer patients in our hospital.Based on pathological diagnosis,76 cases were malignant and 27 cases were benign.Using surgical pathological examination as the gold standard,we compared the sensitivity,specificity,and accuracy of ^(18)F-PSMA-1007 PET/CT with ^(18)F-FDG PET/CT and multiparametric MRI in diagnosing prostate cancer,and analyzed their effectiveness in diagnosing prostate cancer staging.Results Using pathological examination as the gold standard,the sensitivity of ^(18)F-PSMA-1007 PET/CT in diagnosing prostate cancer was 94.74%(72/76),and the specificity for diagnosing prostate cancer was 100%(27/27).The sensitivity of ^(18)F-FDG PET/CT in diagnosing prostate cancer was 89.47%(68/76),and the specificity for diagnosing prostate cancer was 70.37%(19/27).The sensitivity of multi parameter MRI in diagnosing prostate cancer was 84.21%(64/76),and the specificity for diagnosing prostate cancer was 62.96%(17/27).The Kappa values of ^(18)F-PSMA-1007 PET/CT,^(18)F-FDG PET/CT,and multi parameter MRI for diagnosing prostate cancer were 0.904,0.598,and 0.461,respectively.The diagnostic accuracy of ^(18)F-PSMA-1007 PET/CT for prostate cancer was higher than that of ^(18)F-FDG PET/CT and multiparametric MRI,and the misdiagnosis rate was lower than that of ^(18)F-FDG PET/CT and multiparametric MRI.The missed detection rate was lower than that of multiparametric MRI(P<0.05).The diagnosis of extracapsular invasion,seminal vesicle invasion,lymph node metastasis,and bone metastasis using ^(18)F-PSMA-1007 PET/CT was higher than that using ^(18)F-FDG PET/CT and multiparametric MRI(P<0.05).Conclusion ^(18)F-PSMA-1007 PET/CT,^(18)F-FDG PET/CT,and multi parameter MRI have certain value in the diagnosis and staging of prostate cancer,but ^(18)F-PSMA-1007 PET/CT has better accuracy in diagnosing prostate cancer and staging.
