摘要
目的利拉鲁肽联合二甲双胍治疗2型糖尿病(T2DM)的效果。方法选择2020年1月至2023年1月在本院治疗的T2DM患者120例,应用随机数表法将其分为对照组(n=60)及观察组(n=60),对照组接受二甲双胍治疗,观察组在对照组的基础上联合利拉鲁肽治疗,对比两组患者血糖水平[空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)]、微炎症状态[肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、血清淀粉样蛋白A(ASAA)]、相关因子的表达[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞指数(HOMA-β)、空腹胰高血糖素(Glg)]、氧化应激指标[丙二醛(MDA)、脂质过氧化氢(LHP)、谷胱甘肽过氧化物酶(GSH-Px)、总抗氧化能力(T-AOC)]和肠道菌群丰度(双歧杆菌、拟杆菌、肠球菌和肠杆菌)等。结果治疗后,两组血糖均降低,观察组FPG、2hPG、HbAlc低于对照组(P<0.05)。治疗后,两组炎症指标降低,观察组TNF-α、IL-6、ASAA低于对照组(P<0.05)。治疗后,两组HOMA-β升高、Glg、HOMA-IR降低,观察组HOMA-β高于对照组、Glg、HOMA-IR低于对照组(P<0.05)。治疗后,两组MDA、LHP降低、GSH-Px、T-AOC升高,观察组MDA、LHP低于对照组、GSH-Px、T-AOC高于对照组(P<0.05)。治疗后,两组观察组菌群指标均改善,观察组双歧杆菌、拟杆菌高于对照组、肠球菌和肠杆菌低于对照组(P<0.05)。结论二甲双胍联合利拉鲁肽治疗T2DM疾病,可降低血糖水平、微炎症水平,减轻氧化应激反应,作用机制与调节胰岛α和胰岛β细胞功能有关。
Objective To observe the effect of liraglutide combined with metformin in the treatment of type 2 diabetes mellitus(T2DM).Methods A total of 120 T2DM patients treated in Jiyuan Jigang Hospital from January 2020 to January 2023 were selected and randomly divided into control group(n=60)and observation group(n=60)using a random number table.The control group received metformin treatment,while the observation group received liraglutide in addition to metformin.The blood glucose levels[fasting plasma glucose(FPG),2-hour postprandial glucose(2hPG),glycosylated hemoglobin(HbA1c)],micro-inflammatory state[tumor necrosis factor(TNF-α),interleukin-6(IL-6),serum amyloid A(ASAA)],expression of related factors[insulin resistance index(HOMA-IR),pancreaticβ-cell index(HOMA-β),fasting glucagon(Glg)],oxidative stress indicators[malondialdehyde(MDA),lipid hydroperoxide(LHP),glutathione peroxidase(GSH-Px),total antioxidant capacity(T-AOC)],and intestinal flora abundance(Bifidobacteria,Bacteroides,Enterococcus,and Enterobacter)were compared between the two groups.Results After treatment,blood glucose levels in both groups decreased,with FPG,2hPG,and HbA1c in the observation group being lower than those in the control group(P<0.05).Inflammatory indicators decreased in both groups post-treatment,with levels of TNF-α,IL-6,and ASAA in the observation group being lower than those in the control group(P<0.05).After treatment,HOMA-βincreased while Glg and HOMA-IR decreased in both groups;HOMA-βwas higher and Glg and HOMA-IR were lower in the observation group compared to the control group(P<0.05).Levels of MDA and LHP decreased,whereas GSH-Px and T-AOC increased in both groups post-treatment,with the observation group showing lower MDA and LHP levels and higher GSH-Px and T-AOC levels compared to the control group(P<0.05).The abundance of intestinal flora improved in the observation group post-treatment,with higher levels of Bifidobacteria and Bacteroides and lower levels of Enterococcus and Enterobacter compared to the control group(P<0.05).Conclusion The combination of metformin and liraglutide in the treatment of T2DM can reduce blood glucose levels and micro-inflammatory levels,alleviate oxidative stress response,and the mechanism of action is related to the regulation of pancreaticα-cell andβ-cell functions.
