脊柱外科手术机器人在胸腰椎骨折椎弓根螺钉内固定治疗中的运用价值

The application value of spinal surgery robot in the treatment of thoracolumbar fractures with pedicle screw internal fixation

摘要 目的研究脊柱外科手术机器人对胸腰椎骨折椎弓根螺钉内固定患者疗效的影响。方法选择2018年2月至2020年2月于首都医科大学附属北京积水潭医院治疗的258例胸腰椎骨折患者,随机分组,对照组采用传统C臂引导下椎弓根螺钉内固定,观察组采用脊柱外科手术机器人引导下椎弓根螺钉内固定。比较两组围手术期指标、Andrew螺钉CT位置分级以及影像、症状指标。结果两组住院时间、骨折椎体前缘高度比、疼痛视觉模拟评分(VAS)、Cobb角降低差异无统计学意义(P>0.05),观察组手术时间、透视次数均低于对照组,螺钉CT位置分级、置钉准确率高于对照组(P<0.05)。结论在胸腰椎骨折椎弓根螺钉内固定治疗中,脊柱外科手术机器人能够缩短手术及透视时间,提升椎弓根螺钉置钉准确率,具有较高的运用价值。 Objective To study the effect of spinal surgery robot on the efficacy of pedicle screw internal fixation in patients with thoracolumbar fractures.Methods A total of 258 patients with thoracolumbar fractures treated in Beijing Jishuitan Hospital Affiliated to Capital Medical University from February 2018 to February 2020 were randomly divided into two groups.The control group was treated with pedicle screw internal fixation under the guidance of traditional C arm,and the observation group was treated with pedicle screw internal fixation under the guidance of spinal surgery robot.The perioperative indicators,Andrew screw CT position grading,imaging and symptom indicators were compared between the two groups.Results There were no statistically significant differences in the length of hospital stay,the ratio of the anterior height of the fractured vertebral body,the Visual Analogue Scale(VAS)score for pain,and the reduction of the Cobb angle between the two groups(P>0.05).The operation time and fluoroscopy times in the observation group were lower than those in the control group,and the screw CT position classification and screw placement accuracy in the observation group were higher than those in the control group(P<0.05).Conclusion Spine surgery robot can shorten the operation and fluoroscopy time in the treatment of thoracolumbar fractures with pedicle screw internal fixation,and improve the accuracy of pedicle screw placement has high application value.
