血清LMR、hs-CRP及MMP-9与耳带状疱疹病毒感染并发面瘫的相关性研究

Study on the Association of Serum LMR,hs-CRP and MMP-9 with Facial Paralysis in Patients with Herpes Zoster Oticus Infection

摘要 为了分析淋巴细胞计数与单核细胞计数比值(Monocyte to lymphocyte ratio,LMR)、超敏C反应蛋白(Hypersensitive C-reactive protein,hs-CRP)及基质金属蛋白酶-9(Matrix Metalloproteinase 9,MMP-9)与耳带状疱疹病毒感染并发面瘫的相关性,本研究选取2019年1月至2024年1月医院收治的116例耳带状疱疹病毒感染患者,根据面瘫情况将其分为面瘫组(39例)和无面瘫组(77例)。对比两组耳带状疱疹病毒感染患者血清LMR、hs-CRP及MMP-9水平差异,采用皮尔森(Pearson)相关系数分析血清LMR、hs-CRP及MMP-9与耳带状疱疹病毒感染并发面瘫的相关性,采用受试者工作特征(Receiver operating characteristic,ROC)曲线下面积评估血清LMR、hs-CRP及MMP-9对耳带状疱疹病毒感染并发面瘫的诊断效能。结果发现,与无面瘫组比较,面瘫组血清LMR表达水平明显降低,hs-CRP、MMP-9表达水平明显增加(P<0.05);Pearson相关性分析结果显示,血清LMR与耳带状疱疹病毒感染并发面瘫呈负相关(r=-0.705,P<0.05),血清hs-CRP及MMP-9与耳带状疱疹病毒感染并发面瘫呈正相关(r=0.646、0.584,均P<0.05);ROC曲线分析结果显示,血清LMR、hs-CRP与MMP-9各指标单独及联合检测耳带状疱疹病毒感染并发面瘫的相关性的AUC分别是0.937、0.883、0.874和0.983,与各指标单独检测相比,血清LMR、hs-CRP与MMP-9联合检测的AUC最高(Z=2.292、3.095、3.599,均P<0.05)。血清LMR、hs-CRP及MMP-9与耳带状疱疹病毒感染并发面瘫相关,且三者联合检测可提高对耳带状疱疹病毒感染并发面瘫的诊断效能。 This study aimed to investigate the correlations of the lymphocyte-to-monocyte ratio(LMR),highsensitivity C-reactive protein(hs-CRP),and matrix metalloproteinase-9(MMP-9)with facial paralysis complicated by herpes zoster oticus infection.A total of 116 patients diagnosed with herpes zoster oticus infection,admitted between January 2019 and January 2024,were enrolled.Based on the presence of facial paralysis,patients were divided into a facial paralysis group(n=39)and a non-facial paralysis group(n=77).Serum LMR,hs-CRP,and MMP-9 levels were compared between the two groups,and Pearson correlation coefficients were calculated to assess their relationships with facial paralysis.The diagnostic performance of these biomarkers was evaluated using receiver operating characteristic(ROC)curve analysis.Compared with the non-facial paralysis group,the facial paralysis group showed significantly lower serum LMR levels and significantly higher hs-CRP and MMP-9 levels(all P<0.05).Pearson correlation analyses indicated that serum LMR was negatively correlated with facial paralysis(r=-0.705,P<0.05),whereas hs-CRP and MMP-9 were positively correlated(r=0.646 and 0.584,respectively;both P<0.05).ROC curve analyses demonstrated that the areas under the curve(AUCs)for LMR,hs-CRP,and MMP-9 alone were 0.937,0.883,and 0.874,respectively,while their combined detection yielded an AUC of 0.983.This combined detection approach significantly outperforms individual biomarkers(Z=2.292,3.095,3.599;all P<0.05).In conclusion,serum LMR,hs-CRP,and MMP-9 are significantly associated with facial paralysis in patients with herpes zoster oticus infection,and their combined assessment can enhance diagnostic accuracy.
