摘要
目的:探讨急性心力衰竭伴容量超负荷患者早期应用超滤治疗的效果。方法:选取2022年6月—2024年3月喀什地区第一人民医院心脏重症监护室收治的90例急性心力衰竭伴容量超负荷患者为研究对象,经随机数字表法分为非超滤组和超滤组,各45例。非超滤组常规实施利尿剂及对症治疗,超滤组在非超滤组基础上增加早期超滤治疗。对比尿量、体质量、呼吸困难程度、心力衰竭指标水平、心功能、下肢静脉指标。结果:治疗后,两组尿量高于治疗前,体质量和呼吸困难评分低于治疗前,超滤组优于非超滤组(P<0.05);治疗后,两组B型利钠肽水平降低,左室舒张末期内径、左室收缩末期内径减小,左室射血分数升高,超滤组优于非超滤组(P<0.05);治疗后,两组下腔静脉内径减小,下腔静脉塌陷指数降低,超滤组优于非超滤组(P<0.05)。结论:急性心力衰竭伴容量超负荷患者早期应用超滤治疗的效果显著,可减轻液体潴留,缓解呼吸困难症状,缓解心力衰竭,提高心功能,改善下肢循环。
Objective:To investigate the efficacy of early ultrafiltration therapy in patients with acute heart failure and volume overload.Methods:Ninety patients with acute heart failure and volume overload admitted to the Cardiac Intensive Care Unit of Kashgar First People's Hospital from June 2022 to March 2024 were selected as the study subjects.They were randomly divided into a non-ultrafiltration group and an ultrafiltration group,with 45 patients in each group.The non-ultrafiltration group received conventional diuretic and symptomatic treatment,while the ultrafiltration group received early ultrafiltration therapy in addition to the treatment of the non-ultrafiltration group.Urine output,body weight,degree of dyspnea,heart failure indicators,cardiac function,and lower limb venous indicators were compared.Results:After treatment,urine output increased and body weight and dyspnea scores decreased in both groups compared to pre-treatment levels,with the ultrafiltration group showing superior outcomes to the non-ultrafiltration group(P<0.05).After treatment,B-type natriuretic peptide levels decreased,left ventricular end-diastolic diameter and left ventricular end-systolic diameter reduced,and left ventricular ejection fraction increased in both groups,with the ultrafiltration group outperforming the non-ultrafiltration group(P<0.05).After treatment,the inferior vena cava diameter and collapse index decreased in both groups,with the ultrafiltration group showing better results than the nonultrafiltration group(P<0.05).Conclusion:Early application of ultrafiltration therapy in patients with acute heart failure and volume overload has significant efficacy,alleviating fluid retention,relieving dyspnea,mitigating heart failure,improving cardiac function,and enhancing lower limb circulation.
