摘要
目的:观察经肛括约肌间切开(TROPIS)挂线术与常规高位肛瘘挂线术治疗高位复杂性肛瘘的疗效。方法:将2022年4月至2023年4月我院收治的96例高位复杂性肛瘘患者随机分为对照组和观察组,每组48例。对照组患者采用常规高位肛瘘挂线术治疗,观察组患者采用TROPIS挂线术治疗,比较2组患者的疗效、手术相关指标、术后不同时点疼痛评分、手术前后肛门功能各项评分以及术后并发症发生情况。结果:观察组患者治疗总有效率明显高于对照组(P<0.05)。观察组患者手术时间、挂线脱落时间、住院时间、创面愈合时间均短于对照组(P<0.05),瘢痕面积小于对照组(P<0.05)。观察组患者术后1 d、3 d、7 d疼痛评分均低于对照组(P<0.05)。手术前2组患者肛门功能各项评分比较差异无统计学意义(P>0.05),术后观察组患者肛门功能各项评分均低于对照组(P<0.05)。观察组患者术后并发症发生率低于对照组(P<0.05)。结论:相较于传统挂线术,TROPIS挂线术治疗高位复杂性肛瘘效果更好,并且具有术后疼痛轻、并发症少、肛门功能恢复快等优点,值得临床推广应用。
Objective To observe the efficacy of transanal opening of intersphincteric space(TROPIS)and routine thread-drawing surgery in the treatment of high complex anal fistula.Methods A total of 96 patients with high complex anal fistula admitted to our hospital from April 2022 to April 2023 were randomly divided into the control group and the observation group,with 48 cases in each group.Patients in the control group were treated with routine thread-drawing surgery and patients in the observation group received TROPIS.The efficacy,operation-related indicators,postoperative pain scores at different time points,anal function scores before and after surgery,and postoperative complications were compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The operation time,time of thread shedding,hospital stay,and wound healing time of the observation group were shorter than those of the control group(P<0.05),and the scar area was smaller than that of the control group(P<0.05).The pain scores of the observation group were lower than those of the control group 1 d,3 d and 7 d after operation(P<0.05).There was no significant difference in the scores of anal function between the two groups before operation(P>0.05).After operation,the scores of anal function in the observation group were lower than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion TROPIS is more effective than routine thread-drawing surgery in the treatment of high complex anal fistula,and it has the advantages of less postoperative pain,fewer complications,and faster recovery of anal function,which is worthy of clinical application.
