摘要
目的 探讨自制骨复胶囊在股骨颈骨折内固定术后患者中降低股骨头坏死的应用效果。方法 选择行股骨颈骨折内固定术患者128例,采用随机数表法分为对照组与观察组,各64例。对照组予常规治疗,观察组在对照组基础上予自制骨复胶囊治疗。比较2组临床疗效结果,比较2组治疗前后中医证候积分、骨代谢指标、髋关节功能及生活质量变化,比较2组不良反应发生情况。结果 治疗后,观察组中医证候积分低于对照组(P<0.05);观察组血清总Ⅰ型前胶原氨基端延长肽(tPⅠNP)、骨钙素(BGP)、碱性磷酸酶(ALP)、胰岛素样生长因子Ⅰ(IGF-Ⅰ)水平、Harris与SF-36评分高于对照组(P<0.05);观察组临床疗效总有效率(90.63%,58/64)高于对照组(70.31%,45/64)(P<0.05);观察组股骨头坏死发生率低于对照组(P<0.05),2组不良反应情况比较,差异无统计学意义(P>0.05)。结论 自制骨复胶囊能够改善股骨颈骨折内固定术后患者骨代谢指标,降低中医证候积分及股骨头坏死发生率,改善髋关节功能,提高临床疗效及与生活质量,安全性较高。
Objective To explore the application effect of self-made Gufu capsule in reducing femoral head necrosis in patients after internalfixation of femoral neck fracture.Methods A total of 128 patients who underwent internalfixation of femoral neck fracture were selected and divided into the control group and the observation group according to random number table method,with 64 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with self-made Gufu capsule on the basis of the treatment of the control group.The therapeutic effect of two groups was compared.The changes in the traditional Chinese medicine(TCM)syndrome score,bone metabolism index,hip joint function,and quality of life before and after treatment between two groups were compared.The incidence of adverse reactions of two groups was compared.Results After treatment,TCM syndrome scores in the observation group were lower than those in the control group(P<0.05);The levels of serum total typeⅠprocollagen N-terminal extension peptide(tPⅠNP),osteocalcin(BGP),alkaline phosphatase(ALP)and insulin-like growth factorⅠ(IGF-Ⅰ),and Harris score and SF-36 score in the observation group were higher than those in the control group(P<0.05);The total effective rate of the observation group(90.63%,58/64)was higher than that of the control group(70.31%,45/64)(P<0.05);The incidence of femoral head necrosis in the observation group was lower than that in the control group(P<0.05),and there was no statistically significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion The self-made Gufu capsule can improve the levels of bone metabolic indicators of patients after internalfixation of femoral neck fracture,reduce TCM syndrome scores and the incidence of femoral head necrosis,improve hip joint function,and improve clinical efficacy and quality of life.This therapy has good safety.
