摘要
目的:探讨扶正培本祛邪汤联合DCF方案(多西他赛+顺铂+5–氟尿嘧啶)化疗治疗中晚期胃癌的临床效果。方法:选取2019年1月至2022年1月三门峡市中医院收治的80例中晚期胃癌患者,采用随机数字表法将患者分为观察组与对照组,各40例。对照组患者实施DCF方案化疗,观察组在对照组基础上增加扶正培本祛邪汤治疗。比较两组患者客观缓解率(ORR)、疾病控制率(DCR),治疗前后中医证候积分、免疫功能及营养状况指标变化情况。结果:观察组患者ORR、DCR为57.50 %、82.50 %、高于对照组的27.50 %、60.00 %;治疗后两组患者食少纳呆、恶心、腹胀、上腹隐痛或刺痛、面色无华、神疲乏力、便溏不爽中医证候积分均降低,且观察组较对照组低;治疗后两组患者血清免疫球蛋白(Ig)M、IgG、IgA水平均升高,且观察组较对照组高;治疗后两组患者血清总蛋白(TP)、血清白蛋白(Alb)和血红蛋白(Hb)、体质量指数(BMI)均升高,且观察组较对照组高;观察组患者不良反应发生率较对照组低。以上比较差异均具有统计学意义(P<0.05)。结论:扶正培本祛邪汤联合DCF方案化疗治疗中晚期胃癌效果显著,可减轻其中医相关症状,改善免疫功能,提升营养水平,降低化疗不良反应发生率。
Objective To explore the clinical effect of Fuzheng Peiben Quxie decoction combined with DCF (docetaxel + cisplatin +5 fluorouracil) chemotherapy in the treatment of advanced gastric cancer.Methods Eighty patients with advanced gastric cancer admitted to Sanmenxia Hospital of Traditional Chinese Medicine from January 2019 to January 2022 were selected,and the patients were divided into an observation group and a control group by random number table,with 40 patients in each group.Patients in the control group received chemotherapy with DCF regimen,and the observation group received additional treatment with Fuzheng Peiben Quxie decoction on the basis of the control group.The objective response rate (ORR),disease control rate (DCR),and the changes of traditional Chinese medicine (TCM) syndrome score,immune function and nutritional status indicators before and after treatment were compared.Resuls The ORR and DCR in the observation group were 57.50% and 82.50%,respectively,which were higher than those of the control group (27.50% and 60.00%).After treatment,both groups of patients showed a decrease in TCM syndrome scores such as overeating,nausea,bloating,upper abdominal pain or stabbing,pale complexion,fatigue,and loose stools,and the observation group had lower scores than the control group.After treatment,the levels of serum immunoglobulin (Ig) M,IgG,and IgA in both groups of patients increased,and the observation group was higher than the control group.After treatment,the levels of serum total protein (TP),serum albumin (Alb),hemoglobin (Hb),and body mass index (BMI) increased in both groups of patients,and the observation group was higher than the control group.The incidence of adverse reactions in the observation group was lower than that in the control group.The above comparisons show statistically significant differences (P < 0.05).Conclusion The combination of Fuzheng Peiben Quxie decoction and DCF chemotherapy has a significant effect on the treatment of advanced gastric cancer,which can alleviateTCM related symptoms,improve immune function,enhance the patient's nutritional level,and reduce the incidence of chemotherapy adverse reactions.
