摘要
目的探讨彩色多普勒超声检查联合血清糖类抗原125(CA125)、糖类抗原199(CA199)水平检测对子宫肌瘤的诊断价值。方法选取108例子宫肌瘤患者为研究组,108例健康体检者为对照组。对比两组入院时的彩色多普勒超声检查参数(RI、ED、PS)及血清CA125、CA199水平。对比研究组中不同肌瘤数量、直径患者的RI、ED、PS以及血清CA125、CA199水平,分析各指标与肌瘤数量、直径的相关性。绘制受试者操作特性(ROC)曲线,分析RI、ED、PS联合血清CA125、CA199水平对子宫肌瘤的诊断效能。结果研究组的RI、ED、PS以及血清CA125、CA199水平均高于对照组(P<0.05)。研究组中肌瘤数量≥3个患者的RI、PS、ED以及血清CA125、CA199水平高于肌瘤数量<3个患者,肌瘤直径≥5 cm患者的RI、PS、ED以及血清CA125、CA199水平均高于肌瘤直径<5 cm患者(P<0.05)。Spearman相关性分析显示,RI、ED、PS以及血清CA125、CA199水平均与肌瘤数量、肌瘤直径呈正相关(P<0.05)。RI、PS、ED及血清CA125、CA199检测联合诊断子宫肌瘤的效能为0.785,较各指标单独诊断更高。结论彩色多普勒超声检查联合血清CA125、CA199水平检测对子宫肌瘤具有较高的诊断价值。
Objective To explore the diagnostic value of color Doppler ultrasound examination combined with detection of serum carbohydrate antigen 125(CA125)and carbohydrate antigen 199(CA199)levels for uterine fibroids.Methods 108 patients with uterine fibroids were selected as the study group,108 healthy subjects were selected as the control group,and the color Doppler ultrasound examination parameters(RI,ED,PS)and serum CA125 and CA199 levels were compared between the two groups.In the study group,RI,ED,PS,and serum CA125 and CA199 levels were compared between patients with different number of fibroids and between patients with different diameter of fibroids,and the correlations between each index and the number of fibroids and the diameter of fibroids were analyzed.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic efficacy of RI,ED,PS combined with serum CA125 and CA199 levels for uterine fibroids.Results The RI,ED,PS and levels of serum CA125 and CA199 in the study group were higher than those in the control group(P<0.05).In the study group,the RI,ED,PS and levels of serum CA125 and CA199 in patients with fibroid number≥3 were higher than those in patients with fibroid number<3,and the RI,ED,PS and levels of serum CA125 and CA199 in patients with fibroid diameter≥5 cm were higher than those in patients with fibroid diameter<5 cm(P<0.05).Spearman correlation analysis showed that RI,ED,PS and levels of serum CA125 and CA199 were positively correlated with the fibroid number and fibroid diameter(P<0.05).The efficacy of combined detection of RI,PS,ED and serum CA125 and CA199 in the diagnosis of uterine fibroids was 0.785,higher than that of each index alone.Conclusions Color Doppler ultrasound examination combined with detection of serum CA125 and CA199 levels has high diagnostic value for uterine fibroids.
