摘要
目的 观察温脾补肾方治疗非小细胞肺癌(NSCLC)患者口服靶向药奥希替尼所致脾肾阳虚型腹泻的临床效果。方法选取2020年1月-2022年10月于三明市中西医结合医院就诊且符合入选标准的NSCLC患者60例,随机分为对照组和试验组,各30例。对照组予益生菌调节肠道生态平衡,予蒙脱石散、盐酸洛哌丁胺胶囊止泻,病情较重者给予补液、纠正电解质等支持处理;试验组在对照组基础上,联合口服温脾补肾方治疗。两组均于7 d后评估疗效、不良反应及中医证候积分。结果试验组总有效率为83.33%(25/30),高于对照组的60.00%(18/30)(P<0.05)。治疗后,试验组大便泄泻、腹胀腹痛、食欲下降、倦怠乏力、畏寒肢冷中医证候积分均较治疗前改善(P<0.05),且各项积分均低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 温脾补肾方治疗NSCLC患者口服奥希替尼致脾肾阳虚型腹泻效果明显,可改善其中医证候且安全可靠,值得临床推广使用。
Objective To observe the clinical efficacy of warming spleen and tonifying kidney prescription in the treatment of yang de-ficiency of spleen and kidney diarrhea induced by oral osimertinib in non-small cell lung cancer(NSCLC).Methods A total of 60 pa-tients with NSCLC who met the criteria in Sanming Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to October 2022 were randomly divided into two groups,with 30 cases in each group.The control group was given probiotics to regulate intestinal ecological balance,montmorillonite powder and loperamide hydrochloride capsule to stop diarrhea,and those in se-vere condition were given support treatment such as rehydration and electrolyte correction.The experimental group was treated with warming spleen and tonifying kidney prescription on the basis of the control group,and the efficacy was evaluated 7 days later.Re-sults The total effective rate of the experimental group was 83.33%(25/30),which was higher than that of the control group(60.00%,18/30)(P<0.05).After treatment,the scores of diarrhea,abdominal distension and abdominal pain,decreased appetite,fatigue,and cold extremities in the TCM syndrome of the experimental group were all improved compared with the pre-treatment scores(P<0.05),and all scores were lower than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reac-tions between the two groups(P>0.05).Conclusion Warming spleen and tonifying kidney prescription has obvious curative effect in the treatment of NSCLC patients with yang deficiency of spleen and kidney caused by oral osimertinib,can improve TCM syndromes,is safe and reliable,and is worthy of clinical application.
