Mallampati气道Ⅲ-Ⅳ级患者全身麻醉气管插管不同喉镜应用观察

Mallampati Different Laryngoscopic Applications of Endotracheal Intubation under General Anesthesia in Patients with Airway GradeⅢ-Ⅳ

摘要 目的:观察Mallampati气道评估分级为Ⅲ-Ⅳ级患者,在全身麻醉气管插管时分别应用可视喉镜与普通直接喉镜的效果。方法:选取2021年2月~2023年12月在贵州省仁怀市中医院麻醉科行外科手术采用全身麻醉气管插管的患者200例作为研究对象。其中全身麻醉采用普通直接喉镜气管插管的80例为对照组,另外120例全身麻醉采用可视喉镜气管插管设为观察组。比较两组喉镜的插管时间、次数、一次性成功率、是否需要辅助按压;对两组喉镜看到的结构所获得的视野进行的分级(Cormack-Lehane分级)进行比较;比较两组喉镜使用后插管后并发症(黏膜出血、咽痛、声嘶)发生情况。结果:观察组一次插管成功109例,成功率90.83%明显高于对照组的61.25%,比较有差异性(P<0.05)。观察组在插管时间(50.82±5.34)s,插管次数(1.02±0.46)次,辅助按压12.50%显著少于对照组,比较有差异(P<0.05)。按照Cormack-Lehane分级1级和2级为良好,观察组喉镜显露良好率为85.00%,显著高于对照组的46.25%,比较有差异(P<0.05)。观察组在黏膜出血(6.67%)、咽痛(3.33%)、声嘶(1.67%)的发生率明显低于对照组,比较有差异(P<0.05)。结论:在Mallampati气道评估分级为Ⅲ-Ⅳ级的患者,在全身麻醉气管插管时可视喉镜较普通喉镜暴露声门视野更好、插管效率更高、并发症更少。 Objective:To observe the effects of using visual laryngoscope and ordinary direct laryngoscope during general anesthesia tracheal intubation in patients with Malampati airway evaluation gradeⅢ-Ⅳ.Methods:200 patients who underwent surgical procedures using general anesthesia and tracheal intubation at the Department of Anesthesiology,Renhuai City Hospital of Traditional Chinese Medicine,Guizhou Province from February 2021 to December 2023 were selected as the study subjects.Among them,80 cases of general anesthesia were treated with ordinary direct laryngoscopic tracheal intubation as the control group,and 120 cases of general anesthesia were treated with visual laryngoscopic tracheal intubation as the observation group.Compare the intubation time,frequency,one-time success rate,and the need for auxiliary compression between two groups of laryngoscopes;Compare the visual field obtained by comparing the structures observed through two groups of laryngoscopes using the Cormack-Lehane classification;Compare the incidence of complications(mucosal bleeding,sore throat,and hoarseness)after intubation between two groups of laryngoscopes.Results:There were 109 successful intubation cases in the observation group,with a success rate of 90.83%,which was significantly higher than the control group's 61.25%,and there was a significant difference(P<0.05).The observation group had a significantly lower assisted compression rate of 12.50%compared to the control group,with a intubation time of(50.82±5.34)seconds,a intubation frequency of(1.02±0.46)times,and a significant difference(P<0.05).According to the Cormack-Lehane grading system,grades 1 and 2 are considered good.The observation group had a good laryngoscopy exposure rate of 85.00%,which was significantly higher than the control group's 46.25%,with a significant difference(P<0.05).The incidence of mucosal bleeding(6.67%),sore throat(3.33%),and hoarseness(1.67%)in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:In patients with Malampati airway assessment gradesⅢ-Ⅳ,visual laryngoscopy provides better exposure of glottic field,higher intubation efficiency,and fewer complications during general anesthesia tracheal intubation compared to conventional laryngoscopy.
