摘要
目的:探讨基于临床路径的分级护理在骨科患者术后下肢深静脉血栓(DVT)预防中的应用效果。方法:选取2023年4月—2024年8月于徐州市第一人民医院骨科接受手术治疗的84例患者为研究对象,采用随机数字表法分为常规组(n=42)与观察组(n=42)。常规组实施常规护理,观察组实施基于临床路径的分级护理。比较两组护理效果。结果:护理后,两组纤维蛋白原、D-二聚体水平均降低,且观察组低于常规组(P<0.05)。两组护理前后凝血时间、凝血酶原时间、活化部分凝血活酶时间比较,无统计学差异(P>0.05)。护理后,两组生活质量评分均高于护理前,且观察组高于常规组(P<0.05)。观察组下肢并发症总发生率低于常规组(P=0.026)。结论:基于临床路径的分级护理在骨科患者术后DVT预防中的应用效果较好,能够改善患者的凝血功能,降低下肢并发症发生率,提高生活质量。
Objective:To explore the effect of graded nursing based on clinical pathway in preventing postoperative deep vein thrombosis(DVT)in lower limbs of orthopedic patients.Methods:A total of 84 patients who underwent surgical treatment in Xuzhou No.1 People's Hospital from April 2023 to August 2024 were selected as the study subjects.They were divided into a conventional group(n=42)and an observation group(n=42)using a random number table method.The conventional group received conventional nursing,while the observation group received graded nursing based on clinical pathway.The nursing effect was compared between the two groups.Results:After nursing,the levels of fibrinogen and D-dimer in the two groups decreased,and the levels in the observation group were lower than those in the conventional group(P<0.05).There was no statistical difference in coagulation time,prothrombin time and activated partial thromboplastin time between the two groups before and after nursing(P>0.05).After nursing,the quality of life scores in two groups were higher than those before nursing,and the scores in the observation group were higher than those in the conventional group(P<0.05).The total incidence of lower limb complications in the observation group was lower than that in the conventional group(P=0.026).Conclusion:Graded nursing based on clinical pathway is effective in preventing postoperative DVT in lower limbs of orthopedic patients,and can improve coagulation function of patients,reduce the incidence of lower limb complications,and improve the quality of life.
