摘要
目的比较计算机断层扫描(CT)和磁共振成像(MRI)对视网膜母细胞瘤(RB)及其视神经侵犯的诊断价值。方法前瞻性队列研究。纳入濮阳市第二人民医院2021年1月至2024年1月共63例疑诊为RB的患者同时行CT和MRI检查,以病理检查结果为"金标准",比较CT与MRI诊断RB的敏感度及RB视神经侵犯分期符合率。采用Kappa检验分析两种检查的一致性。结果病理诊断显示RB 44例,非RB 19例;CT诊断RB的敏感度为93.18%(41/44),特异度为89.47%(17/19),准确度为92.06%(58/63),Kappa值为0.81;MRI诊断RB的敏感度为75.00%(33/44),特异度为84.21%(16/19),准确度为77.78%(49/63),Kappa值为0.53;CT诊断RB的敏感度、准确率显著高于MRI(χ^(2)=5.44、5.02,P=0.020、0.025),CT与MRI诊断RB特异度比较差异无统计学意义(P=1.000);MRI诊断RB视神经侵犯分期符合率84.85%(28/33),明显高于CT的63.41%(26/41)(χ^(2)=4.26,P=0.039)。结论CT诊断RB准确率较高,但MRI在评估RB视神经侵犯方面具有较大优势。
ObjectiveTo compare the value of computed tomography(CT)and magnetic resonance imaging(MRI)in the diagmosis of retinoblastoma(RB)and its optic nerve invasion.MethodsA total of 63 patients of suspected RB were simultaneously examined with CT and MRI before surgery in the Second People′s Hospital of Puyang from Jan.2021 to Jan.2024 in this prospective cohort study.Pathological examination was used as the gold standard to compare the sensitivity of CT and MRI in diagnosing RB as well as the accordance rate in the staging of RB optic nerve invasion.The consistency of the two methods were analyzed using the Kappa test.ResultsPathological diagnosis showed 44 cases with RB and 19 cases without RB.The sensitivity,specificity,accuracy and Kappa value of CT for diagnosing RB were 93.18%(41/44),89.47%(17/19),92.06%(58/63)and 0.81,respectively.The sensitivity,specificity,accuracy and Kappa value of MRI for diagnosing RB were 75.00%(33/44),84.21%(16/19),77.78%(49/63)and 0.53,respectively.The sensitivity and accuracy of CT for diagnosing RB were significantly higher than those of MRI(χ^(2)=5.44,5.02;P=0.020,0.025),but the difference in specificity showed no statistical significance(P=1.000).The accordance rate of MRI for diagnosing the stage of RB optic nerve invasion was 84.85%(28/33),which was significantly higher than that of CT[63.41%(26/41)](χ^(2)=4.26,P=0.039).ConclusionCT has relatively higher accuracy in diagnosing RB,but MRI has more advantages in evaluating RB optic nerve invasion.
