Er:YAG激光去腐法与传统车针去腐法临床效果对比的Meta分析

摘要 目的:比较铒:钇铝石榴石晶体(Er:YAG)激光去腐与传统车针去腐在临床中应用的效果差异,为不同去腐技术在临床病例中的选择性应用提供循证学参考。方法:计算机检索PubMed、Cochrane library、Web of Science、CNKI、CBM、VIP及万方数据库,查找自1997—2022年发布的Er:YAG激光去腐与传统车针去腐效果比较的随机对照临床研究,由2名评价员根据纳入标准选择文献,提取资料和评价纳入研究方法学质量后,应用Stata 15.0软件进行Meta分析。结果:共纳入12个随机对照研究,共1084例患者。Meta分析结果显示,Er:YAG激光去腐法虽较传统车针去腐法操作时间更长[SMD=1.14,95%CI=(0.42,1.85),P<0.001],但其操作过程与传统车针去腐法相比具有患者疼痛感更轻[SMD=-0.77,95%CI=(-1.07,-0.48),P<0.001],术后1年修复体保留率更高(Z=3.18,P=0.001),修复体边缘密合度更好(Z=2.78,P=0.005)的优势,两者在修复体边缘着色率方面差异无统计学意义(Z=1.51,P=0.130)。结论:Er:YAG激光应用于窝洞预备的临床效果较好,尤其适用于有牙科恐惧症的儿童及成人患者。 Objective:The results of Er:YAG laser and traditional needle removal in clinical application were compared to provide evidence-based reference for the selective application of different removal techniques in clinical cases.Methods:After searching the electronic databases,including PubMed,Cochrane Library,Web of Science,CNKI,CBM,VIP and Wanfang Data,randomized controlled clinical studies comparing the effect of Er:YAG laser and traditional needle decay removal published from 1997 to December 2022 were searched.2 reviewers selected papers according to inclusion criteria,extracted data and evaluated the methodological quality of included studies,and then meta-analysis was performed with Stata 15.0 software.Results:12 RCT studies involved 1084 patients were included in this study.According to the results of Meta-analysis:despite the time of cavity preparation by Er:YAG laser was longer than that of traditional rotary methods[SMD=1.14,95%CI(0.42,1.85),P<0.001].Patients experienced less painful in laser group than those who in the control group[SMD=-1.14,95%CI(-1.99,-0.90),P<0.001].The complete retention rate(Z=3.18,P=0.001)and marginal adaptation(Z=2.78,P=0.005)in laser group was statistically higher than control one.There was no statistically significance in the marginal discoloration between two groups(Z=1.51,P=0.130).Conclusion:Er:YAG laser can effectively reduce the pain of patients during the procedure of caries removal and cavity preparation,especially in patients with anxiety in the dental treatment.
