摘要
目的:对北京地区三级综合性医院呼吸道传染病防治工作的阻碍因素进行分析,并提出相应的建议。方法:对来自北京地区三级综合性医院的传染病防治工作相关负责人开展半结构式访谈,运用扎根理论方法逐级编码访谈资料,探究不同因素间的关系。结果:阻碍北京地区综合性医院呼吸道传染病防治工作的因素包括资源配置制约、传染病防治工作路径阻碍和学科发展路径制约3个主范畴。其中资源配置制约、传染病防治工作路径阻碍、学科发展路径制约直接影响传染病防治工作,通过因果关系路径阻碍传染病防治工作。此外,资源配置制约还通过中介关系路径间接影响传染病防治工作,传染病防治工作路径阻碍通过中介关系及调节关系路径间接阻碍传染病防治工作。结论:北京地区综合性医院呼吸道传染病防治工作受资源配置、传染病防治工作路径和学科发展路径影响,应完善补偿政策,促进资源合理配置,优化传染病防治工作路径,提升传染病防治效率与质量,同时促进学科发展,为应对新发突发传染病疫情提供有效防治方案。
Objective:To analyze the barriers to respiratory infectious disease prevention and control in Grade III general hospitals in Beijing and propose recommendations.Methods:Semi-structured interviews were conducted with infectious disease prevention and control personnel from Grade III general hospitals in Beijing.Data were coded and analyzed using grounded theory to explore the relationships among various factors.Results:Barriers to respiratory infectious disease prevention and control included three main categories:resource allocation constraints,obstructed prevention and control pathways,and disciplinary development limitations.These factors directly hindered prevention and control efforts and also affected them indirectly through causal and intermediary pathways.Resource allocation constraints indirectly influenced prevention and control through mediation,while obstructed prevention and control pathways impeded efforts through mediation and moderation mechanisms.Conclusion:Respiratory infectious disease prevention and control in general hospitals in Beijing is influenced by resource allocation,prevention pathways,and disciplinary development.Improvements should focus on enhancing compensation policies,promoting rational resource allocation,optimizing prevention and control pathways,and advancing disciplinary development to improve efficiency and quality.These measures would better equip hospitals to address emerging infectious disease outbreaks.
