摘要
目的比较关节镜下半月板成形缝合术与半月板部分切除术治疗单纯半月板撕裂伤的临床效果。方法队列研究。抽取2021年7月至2023年7月平煤神马医疗集团总医院收治的单纯半月板撕裂伤患者86例,采用随机数字表法分为部分切除组和成形缝合组,每组43例。部分切除组采用关节镜下半月板部分切除术,成形缝合组采用关节镜下半月板成形缝合术。比较两组疗效、视觉模拟评分法(VAS)评分、美国特种外科医院膝关节量表(HSS)评分、膝关节功能Lysholm量表评分、膝关节活动度及并发症发生情况。结果成形缝合组优良率(93.02%,40/43)高于部分切除组(74.42%,32/43),P<0.05。术后1、3、6 d,成形缝合组VAS评分低于部分切除组(P<0.05)。术后3、6个月,成形缝合组HSS评分、Lysholm评分、膝关节活动度均高于部分切除组(P均<0.05)。成形缝合组并发症发生率(2.33%,1/43)与部分切除组(9.30%,4/43)比较差异未见统计学意义(P>0.05)。结论与半月板部分切除术相比,关节镜下半月板成形缝合术可提高单纯半月板撕裂伤患者膝关节功能,减轻疼痛,安全性良好。
Objective To compare the application effects of arthroscopic meniscus plasty and suture versus meniscus partial resection in the treatment of simple meniscus tears.Methods A total of 86 patients with simple meniscus tear admitted to General Hospital of Pingmei Shenma Medical Group from July 2021 to July 2023 were selected for the cohort study,and they were divided into partial resection group and plasty-suture group by random number table method,with 43 cases in each group.The partial resection group was treated by arthroscopic partial resection of meniscus,and the plasty-suture group was treated by arthroscopic meniscus plasty and suture.The curative effect,visual analogue scale(VAS)score,Hospital for Special Surgery knee scale(HSS)score,Lysholm knee function scale score,range of motion of knee joint and incidence of complication were compared between the two groups.Results The excellent and good rate of the plasty-suture group(93.02%,40/43)was higher than that of the partial resection group(74.42%,32/43),P<0.05.On 1,3 and 6 days after operation,the VAS scores of the plasty-suture group were lower than those of the partial resection group(P<0.05).The HSS score,Lysholm score and range of motion of knee joint in the plasty-suture group were higher than those in the partial resection group 3 and 6 months after operation(all P<0.05).There was no significant difference in the incidence of complications between the plasty-suture group(2.33%,1/43)and the partial resection group(9.30%,4/43),P>0.05.Conclusions Compared with arthroscopic partial resection of meniscus,arthroscopic meniscus plasty and suture in the treatment of simple meniscus tear can improve the knee function,and reduce pain,with good safety.
