非小细胞肺癌组织ADGRD1、PTK7表达水平及与淋巴结转移和预后的关系研究

Study on the expression level of ADGRD1,PTK7 in non-small cell lung cancer and its relationship with lymph node metastasis and prognosis

摘要 目的探讨非小细胞肺癌(NSCLC)组织中的黏附G蛋白耦联受体D1(ADGRD1)、蛋白酪氨酸激酶7(PTK7)表达水平,及其与淋巴结转移和预后的关系。方法收集邯郸市中心医院2019年8月至2020年8月收治的152例NSCLC患者的癌组织及癌旁正常余肺组织(距肿瘤切缘≥2 cm)。采用免疫组织化学法检测癌组织及癌旁正常余肺组织中ADGRD1、PTK7表达水平。分析ADGRD1、PTK7表达与NSCLC患者临床病理特征的关系。采用Spearman检验分析癌组织中ADGRD1、PTK7表达与淋巴结转移的相关性,采用多因素Logistic回归模型分析NSCLC患者淋巴结转移的影响因素。采用Spearman检验分析癌组织中ADGRD1表达和PTK7表达之间的相关性。随访3年,通过Cox回归模型以及Kaplan-Meier生存曲线分析NSCLC患者预后情况。结果与癌旁正常余肺组织比较,癌组织ADGRD1阳性表达率显著降低(P<0.05),PTK7阳性表达率显著升高(P<0.05)。发生淋巴结转移、低分化程度、TNM分期Ⅲa期NSCLC患者的ADGRD1阳性表达率显著低于未发生淋巴结转移、中高分化程度、TNM分期Ⅰ+Ⅱ期NSCLC患者(P<0.05)。出现淋巴结转移以及TNM分期Ⅲa期者的PTK7阳性表达率高于未见淋巴结转移以及TNM分期Ⅰ+Ⅱ期者(P<0.05)。ADGRD1阴性表达、PTK7阳性表达时,NSCLC患者的淋巴结转移明显增加(P<0.05)。PTK7阳性表达以及ADGRD1阴性表达均为NSCLC淋巴结转移的危险因素(P<0.05)。152例中最终146例获得随访,ADGRD1阴性表达组3年总生存率低于ADGRD1阳性表达组(P<0.05);PTK7阳性表达组3年总生存率低于PTK7阴性表达组(P<0.05)。淋巴结转移、ADGRD1阴性表达、TNM分期Ⅲa期、PTK7阳性表达均为NSCLC不良预后的危险因素(P<0.05)。癌组织中ADGRD1表达水平与PTK7表达水平呈负相关(P<0.05)。结论NSCLC患者癌组织中ADGRD1呈低表达、PTK7呈高表达,与淋巴结转移、不良预后结局的发生有关。 Objective To investigate the expression levels of adhesion G protein coupled receptor D1(ADGRD1)and protein tyrosine kinase 7(PTK7)in non-small cell lung cancer(NSCLC)tissues and their relationship with lymph node metastasis and prognosis.Methods One hundred and fifty-two NSCLC patients who were admitted to Handan Central Hospital from August 2019 to August 2020 were selected,and cancer tissues and cancer normal peripheral lung tissue(distance from tumor margin≥2 cm)were collected.ADGRD1 and PTK7 expression levels in cancer tissues and cancer normal peripheral lung tissue were detected by immunohistochemical technique.The relationship between ADGRD1 expression,PTK7 expression and the clinicopathological characteristics of NSCLC patients were analyzed.The correlation between ADGRD1 expression and PTK7 expression in cancer tissues and lymph node metastasis were analyzed by Spearman test.The influencing factors of lymph node metastasis in NSCLC patients were analyzed by multivariate Logistic regression model.The correlation between ADGRD1 expression and PTK7 expression in cancer tissues were analyzed by Spearman test.After 3 years of follow-up,the prognosis of NSCLC patients was analyzed by Cox regression model and Kaplan-Meier survival curve.Results Compared with cancer normal peripheral lung tissue,ADGRD1 positive expression rate in cancer tissues was significantly decreased(P<0.05),and PTK7 positive expression rate was significantly increased(P<0.05).ADGRD1 positive expression rate in patients with lymph node metastasis,poorly differentiated and TNM stageⅢa NSCLC was significantly lower than that in patients with no lymph node metastasis,middle to highly differentiated and moderately and TNM stageⅠ+ⅡNSCLC(P<0.05).PTK7 positive expression rate in patients with lymph node metastasis and TNM stageⅢa was higher than that in patients with no lymph node metastasis and TNM stageⅠ+Ⅱ(P<0.05).When ADGRD1 was negative and PTK7 was positive,the lymph node metastasis of NSCLC patients was significantly increased(P<0.05).PTK7 positive expression and ADGRD1 negative expression were risk factors for lymph node metastasis in NSCLC(P<0.05).Of the 152 cases,146 were finally followed up.The 3-year overall survival rate in ADGRD1 negative expression group was significantly lower than that in ADGRD1 positive expression group(P<0.05).The 3-year overall survival rate in PTK7 positive expression group was significantly lower than that in PTK7 negative expression group(P<0.05).Lymph node metastasis,ADGRD1 negative expression,TNM stageⅢa and PTK7 positive expression were risk factors for poor prognosis of NSCLC(P<0.05).ADGRD1 expression level in cancer tissues was negatively correlated with PTK7 expression level(P<0.05).Conclusions Low expression of ADGRD1 and high expression of PTK7 in cancer tissues of NSCLC patients are related to lymph node metastasis and poor prognosis.
