摘要
目的探讨在复杂软组织缺损修复中运用游离股前外侧皮瓣的修复效果及对术后功能的影响,为临床修复工作提供参考。方法选取2020年1月至2024年1月许昌仁和骨伤医院收治的足部创伤性复杂软组织缺损患者,其中26例患者采取游离股前外侧皮瓣修复治疗作为观察组,26例患者采取植皮联合负压引流修复治疗作为对照组。对比分析各组修复情况、足部功能、瘢痕情况、并发症情况。结果观察组患者住院时间、Ⅰ期愈合时间均短于对照组,差异有统计学意义(P<0.05)。观察组皮瓣成活率100.00%、患者满意度96.15%高于对照组,差异有统计学意义(P<0.05)。观察组患者术后1个月、2个月、3个月和4个月足部功能评分均高于对照组,差异具有统计学意义(P<0.05)。观察组患者瘢痕弹性、宽度、颜色、平整度评分低于对照组,差异具有统计学意义(P<0.05)。观察组术后并发症发生率3.85%低于对照组23.08%,差异具有统计学意义(P<0.05)。结论在该类足部创伤修复中,采用游离股前外侧皮瓣的成活率更高,瘢痕增生程度相对较轻,并可保证外观美观,同时还可降低并发症风险,缩短愈合时间,促进足部功能恢复,修复效果更为满意。
Objective Exploring the repair effect and postoperative function of using free anterior lateral thigh skin flap in complex soft tissue defect repair,providing reference for clinical repair work.Methods Select patients with complex soft tissue defects of foot trauma admitted to Renhe Orthopedic Hospital in Xuchang from January 2020 to January 2024.Among them,26 patients were treated with free anterior lateral thigh skin flap repair as the observation group,and 26 patients were treated with skin grafting combined with negative pressure drainage repair as the control group.Compare and analyze the repair status,foot function,scar condition,and complications of each group.Results The observation group had shorter hospitalization time,and stage I healing time than the control group,and the difference was statistically significant(P<0.05).The survival rate of skin flaps in the observation group was 100.00%,and the patient satisfaction rate was 96.15%,which was higher than that in the control group,and the difference was statistically significant(P<0.05).The foot function scores of the observation group patients were higher than those of the control group at 1,2,3,and 4 months after surgery,and the difference was statistically significant(P<0.05).The scar elasticity,width,color,and flatness scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 3.85%,significantly lower than that in the control group 23.08%,and the difference was statistically significant(P<0.05).Conclusions In this type of foot trauma repair,the use of free anterior lateral thigh skin flap has a higher survival rate,relatively mild scar hyperplasia,and can ensure aesthetic appearance.At the same time,it can reduce the risk of complications,shorten healing time,promote foot function recovery,and achieve more satisfactory repair results.
