摘要
化脓性骨髓炎属于中医学“附骨疽”“附骨痈”等范畴,文章通过系统梳理中国古代文献典籍,总结探析从先秦至明清历代医家对于化脓性骨髓炎病因病机的认识,秦汉时期多以“内虚”立论,外感热邪而致病;晋唐时期,认为患者体质素热,风寒外袭未愈,复感湿邪而致病。宋元时期认为,由于脾肾亏虚、外感风、寒、湿、火等邪气,最终导致以痰积、瘀血为病理表现。明清时期,认为病因主要由于肾、脾、肝亏虚,导致风、寒、暑、湿、火侵入,而致此病。最终为现代临床治疗化脓性骨髓炎提供中医辨治思路。
Purulent osteomyelitis belongs to the categories of“attached bone necrosis”and“attached bone abscess”in traditional Chinese medicine.This article systematically reviews ancient Chinese literature and analyzes the understanding of the etiology and pathogenesis of purulent osteomyelitis by medical practitioners from the pre Qin to the Ming and Qing dynasties.During the Qin and Han dynasties,the theory was mostly based on“internal deficiency”and the disease was caused by external heat pathogens;During the Jin and Tang dynasties,it was believed that patients had a constitution that was hot and had not yet recovered from the external invasion of wind and cold,and were susceptible to dampness and pathogenic factors.During the Song and Yuan dynasties,it was believed that due to spleen and kidney deficiency,external factors such as wind,cold,dampness,and fire,the pathological manifestations were ultimately phlegm accumulation and blood stasis.During the Ming and Qing dynasties,it was believed that the main cause of this disease was due to deficiencies in the kidneys,spleen,and liver,leading to the invasion of wind,cold,heat,dampness,and fire.Ultimately,it provides traditional Chinese medicine diagnosis and treatment ideas for the modern clinical treatment of pyogenic osteomyelitis.
