摘要
目的探讨卵巢硬化性间质瘤(OSST)的磁共振成像表现及其与病理的相关性,以期提高对其认识。方法回顾性分析福建医科大学附属漳州市医院2016年5月至2024年8月经术后病理证实的15例OSST患者,观察OSST患者肿块的大小、形态、境界、囊实性、平扫信号及强化特点等,并与病理结果相对照。结果15例均为单侧;10个呈椭圆形,3个圆形,1个呈分叶状,1个呈不规则状;8个为实性肿块,7个为囊实性肿块,4个囊实性肿块呈湖岛征。磁共振成像增强扫描肿瘤实质呈现3种强化方式:(1)动脉期较均匀显著强化,延迟期呈渐进性持续强化(5例);(2)动脉期周缘显著强化,静脉和延迟期呈渐进性、向心性强化,类似肝脏海绵状血管瘤样强化(6例);(3)动脉期呈较均匀轻度强化,延迟期呈渐进性明显强化(1例)。结论卵巢硬化性间质瘤磁共振成像表现具有一定特征性,结合患者临床表现,能提高其诊断准确率。
Objective To explore the MRA of Ovarian Sclerosing Stromal Tumor(OSST)and its correlation with pathological findings,in order to enhance the understanding and diagnostic ac curacy of this condition.Methods A retrospective analysis was conducted on 15 patients with OSST,confirmed by postoperative pathology,who were treated at Zhangzhou Hospital of Fujian Medical University between May 2016 and August 2024.The size,shape,border,cystic or solid nature,plain scan signals,and enhancement characteristics of the tumors were observed and compared with pathological results.Results All 15 cases were unilateral.Of the tumors,10 were oval-shaped,3 were round,1 was lobulated,and 1 was irregular in shape.Eight tumors were solid,while 7 were cystic-solid,and 4 cystic-solid tumors exhibited a“lake-island”sign.On MRI contrast-enhanced scans,three distinct patterns of enhancement were observed in the tumor parenchyma:In the arterial phase,there was a significantly uniform enhancement,followed by progressive and sustained enhancement in the delayed phase(5 cases).In the arterial phase,peripheral enhancement was prominent,with progressive and centripetal enhancement in the venous and delayed phases,resembling the enhancement pattern of hepatic cavernous hemangiomas(6 cases).In the arterial phase,mild uniform enhancement was observed,with progressive and significant enhancement in the delayed phase(1case).Conclusion The MRI features of ovarian sclerosing stromal tumors are relatively distinctive.When combined with the clinical presentation,MRI can improve the accuracy of diagnosis.
