摘要
目的分析心血管重症患者急性皮肤衰竭的临床特征和预后。方法回顾性纳入厦门大学附属心血管病医院冠心病监护病房2017年1月至2023年2月发生急性皮肤衰竭的患者,对患者的一般资料、出现皮肤衰竭前1周内的乳酸水平、合并疾病、伤口特征及预后进行分析。结果研究期间共13例患者发生急性皮肤衰竭(ASF),出现ASF前1周内乳酸水平升高13例、占比100%,合并疾病中合并肾功能衰竭12例、占比92.31%,合并呼吸衰竭11例、占比84.62%,合并心力衰竭13例、占比100%,合并动脉疾病2例、占比15.38%,合并营养不良11例、占比84.62%,合并脓毒症7例、占比53.85%;使用机械通气11例、占比84.62%,血管手术2例、占比15.38%,使用血管活性药8例、占比61.54%;患者结局1例好转出院、1例转院治疗无法追踪、11例临床死亡,死亡率为84.62%。结论急性皮肤衰竭好发于灌注不足的心血管重症患者,是心血管重症患者预后不良的先兆。
Objective To retrospectively analyze the occurrence and influencing factors of acute skin failure in severe cardiovascular patients,and provide ideas for prevention and care of such patients.Methods A total of 13 cases of acute skin failure in the coronary heart disease intensive care unit of a tertiary cardiovascular hospital from January 2017 to February 2023 were retrospectively collected and analyzed.The general data of patients,lactic acid level in the first week before skin failure,complications,wound characteristics and prognosis of patients were analyzed.Results During the study period,among the 13 patients,lactic acid level increased in 13 cases within one week before ASF,and 12 cases were complicated with renal failure,11 cases with respiratory failure,13 cases with heart failure,2 cases with arterial disease,11 cases with malnutrition,7 cases with sepsis,11 cases with mechanical ventilation,2 cases with vascular surgery and 8 cases with vasoactive drugs.One patient was cured,one patient could not follow the prognosis,and 11 patients died clinically,with a mortality rate of 84.62%.Conclusions Acute skin failure often occurs in cardiovascular patients with insufficient perfusion,which is a precursor to poor prognosis of cardiovascular patients with severe diseases.
