摘要
消化内镜全景全息技术极大地提高了消化系统疾病的临床诊治率,为循证医学提供强力支撑,同时也是中医四诊的延伸,为中医遣方用药提供依据。通过全景全息技术收集临证数据图像,已不仅局限于消化道黏膜色泽和血管粗细等简单认知,而是可关注消化道分泌物,如涎沫、胆汁、血液残留,潴留物的量、色、质及停留部位等的变化。研究从应重视消化道动态变化过程,用发展的眼光总结疾病的发展变化;注重消化道微观下血管、结构的细微变化;用整体的眼光关注消化道不同部位及消化道内与消化道外甚至全身的差异,充分辨析疾病的发展规律,合理用药。研究从全景全息技术下收集的真实世界数据延伸了中医的诊断及疗效观察,使中医诊疗从“经验”向“客观数据”转变,帮助形成一套科学有效的诊疗效果评价体系。客观数据支撑下的疗效评价机制推动了中医科研的发展,将中医真实世界证据应用到中医真实世界研究中去,使中医科研更贴近临床诊疗环境,助力中医科研走出一条中医特色的科研道路。
The panoramic holographic technology of digestive endoscopy greatly improves the clinical diagnosis and treatment rate Tof digestive diseases,provides strong support for evidence-based medicine,and is also an extension of the four diagnostic methods of traditional Chinese medicine,which provides a basis for Chinese medicine prescriptions and medications.The clinical data and images collected by panoramic holographic technology are not limited to simple cognition such as the color of digestive tract mucosa and the thickness of blood vessels,but pay attention to the changes in the amount,color,quality and retention of digestive tract secretions such as saliva,bile,blood residue and retention site.It pays attention to the dynamic change process of digestive tract,and summarizes the development and change of dis-eases from the perspective of development;It pays attention to the microscopic changes of blood vessels and structure of digestive tract;Similarly,we should look at the problem from a holistic perspective,focusing on different parts of the digestive tract and the differences between the inside and outside of the digestive tract and even the whole body to fully identify the developmental pattern of the disease and rational use of medication.The collected data and images are combined with TCM syndrome differentiation,and a large database of TCM diagnosis and treatment is estab-lished through multidisciplinary and multi-technical cooperation to summarize and formulate intervention strategy of“prevention before disease”for digestive diseases s such as early cancers of the digestive tract,inflammatory bowel disease,portal hypertension,etc.,and improve clinical diagnosis and treatment norms.At the same time,the real world data collected under the panoramic holographic technology extends the diagnosis and curative effect observation of traditional Chinese medicine,transforming the diagnosis and treatment of traditional Chinese medicine from“experience”to“objective data”,and helping to form a scientific and effective diagnosis and treatment effect evaluation system.The therapeutic effect evaluation mechanism supported by objective data promotes the development of TCM scientific research,applies real-world evidence of TCM to real-world research of TCM,makes TCM scientific research closer to the clinical diagnosis and treatment environment,and helps TCM scientific research to walk out of a scientific research road with TCM characteristics.
