摘要
目的:探讨肝细胞肝癌及肝血管瘤的彩超检查及鉴别诊断价值。方法:回顾性选取68例本院2021年7月~2023年7月确诊的肝细胞肝癌患者作为肝癌组,另选取68例同期肝血管瘤患者作为肝血管瘤组,分别行彩色多普勒超声和CT检查,比较两组诊断结果及诊断效能。结果:与肝血管瘤比较,肝癌组病灶边界清晰、形态规则、内部回声均匀占比较低,有占位效应占比较高(P<0.05);肝癌组内部及周边血流丰富肿块较高(P<0.05);肝癌组肝门静脉流速较低,肝固有动脉最小流速、肝固有动脉峰值流速较高(P<0.05);超声对于肝细胞肝癌及肝血管瘤的诊断灵敏度和准确度高于常规CT,漏诊率低于常规CT(P<0.05)。结论:彩色多普勒超声可用于肝细胞肝癌及肝血管瘤鉴别诊断中,临床可通过超声影像学特征、血流状况辅助早期诊断,以提高诊断准确率。
Objective:To investigate the value of color ultrasonography in the differential diagnosis of hepatocellular carcinoma(HCC)and hepatic hemangioma.Methods:68 patients with hepatocellular carcinoma diagnosed in our hospital from July 2021 to July 2023 were retrospectively selected as liver cancer group,and another 68 patients with hepatic hemangioma at the same period were selected as liver hemangioma group.Color Doppler ultrasonography and CT examination were performed respectively to compare the diagnostic results and diagnostic efficacy of the two groups.Results:Compared with hepatic hemangioma,the proportion of clear boundary,regular shape and uniform internal echo was lower in HCC group,and the proportion of occupying effect was higher(P<0.05).The internal and peripheral blood flow abundances were higher in HCC group(P<0.05).Hepatic portal vein flow velocity was lower in HCC group,and the minimum flow velocity and peak flow velocity of proper hepatic artery were higher(P<0.05).The diagnostic sensitivity and accuracy of ultrasound for hepatocellular carcinoma and hepatic hemangioma were higher than those of conventional CT,and the rate of missed diagnosis was lower than that of conventional CT(P<0.05).Conclusion:Color Doppler ultrasound can be used in the differential diagnosis of hepatocellular carcinoma(HCC)and hepatic hemangioma.Ultrasound imaging features and blood flow status can be used to assist early diagnosis to improve diagnostic accuracy.
