摘要
目的:系统评价蛭蛇通络胶囊联合常规药物治疗脑梗死的疗效和安全性。方法:在中国知网、万方、维普、PubMed、CBM、Web of Science和ScienceDirect数据库中检索从各数据库建库至2023年12月,关于蛭蛇通络胶囊联合常规药物治疗脑梗死的临床随机对照研究文献。根据纳入和排除标准筛选文献,然后使用Review Manager 5.4软件对数据进行Meta分析。结果:纳入17项研究,共2255例患者。Meta分析结果显示:蛭蛇通络胶囊联合常规药物治疗脑梗死,在提高临床总有效率和Barthel指数,降低NIHSS评分、血液沉降率、血小板最大聚集率、纤维蛋白原、全血粘度、甘油三酯、中医症候积分和延长凝血酶原时间方面优于单纯常规用药,差异具有统计学意义。但在提高高密度脂蛋白,降低血小板计数、血小板最大聚集时间、总胆固醇、低密度脂蛋白和不良反应方面,试验组和对照组差异无统计学意义。结论:相较于常规药物治疗,蛭蛇通络胶囊联合用药治疗脑梗死具有更好的临床疗效。但目前研究的样本量小,且质量普遍不高。因此,所得结论还需更多大样本、高质量的临床研究来进一步验证。
Objective To evaluate the efficacy and safety of combining Zhishe Tongluo Capsules with conventional drugs in the treatment of cerebral infarction.Methods A comprehensive search of CNKI,Wanfang,VIP,PubMed,CBM,Web of Science,and ScienceDirect database was conducted to collect the randomized controlled trials of combining Zhishe Tongluo Capsules with conventional drugs for the treatment of cerebral infarction,with a search period from the establishment of each database to December 2023.Two researchers independently conducted literature screening and data extraction,and then Review Manager 5.4 software was used for meta-analysis.Results Including 17 randomized controlled trials,covering 2255 cases.The meta-analysis results showed that the combination of Zhishe Tongluo Capsules and conventional drugs in the treatment of cerebral infarction improved the clinical total effective rate and Barthel index,reduced NIHSS score,blood sedimentation rate,maximum platelet aggregation rate,fibrinogen,whole blood viscosity,triglycerides,traditional Chinese medicine syndrome accumulation score,and prolonged prothrombin time,with statistically significant differences.However,there was no statistically significant difference between the experimental group and the control group in terms of increasing high-density lipoprotein,reducing platelet count,maximum platelet aggregation time,total cholesterol,low-density lipoprotein,and adverse reactions.Conclusion Based on conventional treatment,the combination of Zhishe Tongluo Capsules shows promise in improving the clinical efficacy of cerebral infarction and is considered safe.But the sample size of the included literature is small and the quality is generally not high.Therefore,the effectiveness and safety of combination therapy require further evaluation through large-scale,high-quality clinical randomized controlled trials.
