摘要
目的 调查重庆市某三甲医院老年医学科护士的共情疲劳现状,并分析其影响因素,构建预测模型。方法 纳入老年医学科39名护士作为研究对象,收集一般资料,通过中文版共情疲劳量表、大五人格量表和社会支持评定量表,评估护士的共情疲劳状况、大五人格特质及社会支持情况,并构建Logistic回归模型。结果 39名护士中,31名存在共情疲劳,发生率为79.49%。单因素分析表明,平均每日工作时间、收入满意度、工作满意度和睡眠质量对护士共情疲劳的发生具有显著影响(P<0.05)。相关性分析显示,责任感人格与共情满意得分呈显著正相关,神经质人格则呈显著负相关。多因素回归分析进一步证实神经质人格与共情满意之间的显著相关性,并建立了回归方程模型:共情满意得分=28.104-4.111×平均每天工作时间-2.171×收入满意程度-2.83×工作满意程度-0.226×睡眠质量-0.452×神经质得分+0.721×责任感得分+0.176×主观支持得分+0.684×支持的利用度。结论 神经质是老年医学科护士共情疲劳的独立影响因素。构建的Logistic回归模型为共情疲劳的管理提供了有效参考,并为临床早期识别护士共情疲劳风险提供了科学依据。
Objective To investigate the current status of compassion fatigue among nurses in the geriatrics department of a tertiary hospital in Chongqing,analyze its influencing factors,and construct a predictive model.Methods A total of 39 nurses from the geriatrics department were enrolled as study participants.General demographic data were collected,and the Chinese versions of the Compassion Fatigue Scale,Big Five Personality Scale,and Social Support Rating Scale were used to assess the nurses' compassion fatigue,Big Five personality traits,and social support.A logistic regression model was constructed to analyze the data.Results Among the 39 nurses,31 experienced compassion fatigue,with an incidence rate of 79.49%.Univariate analysis revealed that average daily working hours,income satisfaction,job satisfaction,and sleep quality had significant impacts on the occurrence of compassion fatigue(P<0.05).Correlation analysis showed that compassion satisfaction scores were positively correlated with conscientiousness and negatively correlated with neuroticism.Multiple regression analysis confirmed the significant relationship between neuroticism and compassion satisfaction.The regression equation was established as follows:Compassion Satisfaction Score=28.104-4.111×Average Daily Working Hours-2.171×Income Satisfaction-2.83×Job Satisfaction-0.226×Sleep Quality-0.452×Neuroticism Score+0.721×Conscientiousness Score+0.176×Subjective Support Score +0.684×Support Utilization.Conclusion Neuroticism is an independent influencing factor of compassion fatigue among nurses in the geriatrics department.The constructed logistic regression model provides valuable reference for the management of compassion fatigue and offers new scientific evidence for the early clinical identification of nurses at risk of compassion fatigue.
